Analysis Of The Correlation Between The 21-gene Recurrence Score And Lymph Node Metastasis In Breast Cancer

Objective:To understand the expression and distribution of 21-gene RS in breast cancer patients in Guangxi,to analyze the correlation between 21-gene RS and axillary lymph node metastases,and to explore the influence of 21-gene RS on making adjuvant treatment decisions.Methods : One hundred and seven breast cancer patients with ER(+),HER-2(-)who had performed surgical treatment were retrospectively recruited from February 2017 and March 2018 at the First Affiliated Hospital of Guangxi Medical University and the Second People's Hospital of Nanning.Among them,there are 101 cases of gastroenterology& gland surgery in the First Affiliated Hospital of Guangxi Medical University and 6 cases of thyroid&breast surgery in the Second People's Hospital of Nanning.Using the DNA-liquid chip technology to carry out 21-gene detection and collect clinical pathological data of patients.Kruskal-Wallis test was used to compares continuous variables,Chi-square test and other statistical methods were used for processing classification variables.Results:1.The RS scores of 107 breast cancer patients ranged from 1 to 50,the median RS score was 16,and the number of patients categorized as low-risk,intermediate-risk,and high-risk was 60(56.1%),32(29.9%)and 15(14.0%)respectively.2.The median RS scores of stage N1,N2,and N3 in lymph node staging were 16.0,16.5,and 18.0,respectively.There was no correlation between lymph node metastasis and RS scores.3.The RS score was related to the histological grade,PR status and expression of Ki-67 in the whole group(all P<0.05).4.26.2% of patients had a change in treatment decisions after knowing the RS results,all occurred in patients with negative lymph nodes,and the change was mainly from endocrine therapy+chemotherapy to simple endocrine therapy.Conclusion: There is no correlation between 21-gene RS and lymph node metastasis,but 21-gene RS was related to histological grading,PR status and ki-67 expression.21-gene RS does not provide additional information for the prediction of lymph node metastasis.21-gene RS may influence decision making for adjuvant treatment in ER(+),HER-2(-),and LN(-)breast cancer patients in this region.
