Objective:To explore the clinical value of preoperative peripheral blood neutrophils and lymphocyte ratio(NLR)and platelet and lymphocyte ratio(PLR)as a new serum marker in the prediction of colorectal cancer(colorectal cancer,CRC)TNM staging.Methods:A retrospective analysis was made of 1210 cases of CRC patients undergoing radical resection of colorectal cancer from January 1,2010 to October 31,2016.The postoperative pathological stages accounted for 12.6%(152 cases),46.3%(560 cases)in stage ?,34.9%in stage ?(422 cases),and 6.2%in stage IV(76 cases).Preoperative hematological count was obtained from the first routine blood test after admission.At the same time,the relative blood cell counts of 493 healthy and healthy people matched by sex and age were selected as control group.The Kruskel Wallis nonparametric test was used to test the differences of NLR and PLR in different stages of colon cancer.The significant level of multiple groups was adjusted by the Bonferroni correction method.The cut-off values,sensitivity and specificity of different stages of colon cancer were calculated by using the Receiver-Operating Characteristic(ROC)curve analysis.Results:There was no significant difference in age and sex factor analysis between the CRC group and the healthy control group(P>0.05);NLR was associated with local infiltration in CRC patients,and the higher NLR level before operation showed that the patients tended to have a late pT stage;PLR was significantly associated with positive lymph node metastasis in CRC patients.Preoperative low PLR level was higher and PLR level tended to have more positive lymph node metastasis.In addition,the area under the ROC curve(AUC)of the NLR was 0.762(95%CI,0.739-0.785;sensitivity 66.7%,specificity 75.3%;P<0.0001),and the ALR of the PLR was 0.621(95%CI,0.594-0.647;Sensitivity 32.6%,specificity 92.7%;P<0.0001).Conclusion:Preoperative NLR and PLR were significantly associated with TNM staging in patients with CRC.Compared with PLR,NLR is a more valuable serum biomarker for predicting CRC staging.
