BackgroundDepression is a high morbidity, high disability, high suicide rate of mental illness, and seriously affects the human body and mind health. It has been generally recognized that it has severe cognitive impairment Attention, the first step of the cognitive process, seriously affects the cognitive process. The study on attentional bias to emotional faces of patients with first depressive disorder, can help to understand the pathogenesis of depression, and provide help for the treatment,so it is worth discussing and studying.ObjectiveThrough the eye movement experiment, studying whether the patients with the first depressive disorder have the attentional bias to emotional faces and comparing the differences before and after treatment between them and normal people,in order to know the features of attentional bias in depressive disorder, and provide evidencefor revealing the mechanism of attentional bias and reference for the diagnosis and treatment of the depressive disorder.MethodsThirty patients with the first depressive disorder, conforming to ICD-10 depression, were selected in Institute of Mental Disease Control and Prevention of Ninety-first Central Hospital from November 2015 to May 2017.All were under treatment of venlafaxine hydrochloride sustained-release tablets for 4 weeks.Meanwhile,30 healthy volunteers around the Physical Examination Center of the hospital were selected as the control group. The social demographic information of the treatment group and the control groupwas recorded by general situation record registration form, including gender, age and length of education. The efficacy of treatment in patients with depressive disorder was evaluated by Hamilton Depression Scale(HAMD)with HAMD<7 points for recovery,HAMD reduction rate?50%for being effective.Negative cognitive bias in patients with the first depression disorder was assessed by negative cognitive processing bias questionnaire and compared with the control group. At the same time,we selected happy, sad, neutral face pictures of facial expression system CAS-PEAL dataset established by CAS Key Laboratory of Intelligent Information Processing as the stimulation material to test the Duration Before,First Fixation Duration,Fixation Count, and Fixation Time,it takes the subjects reached the region of interest, and extract the attention bias score, attention bias fraction, duration bias percentage before reaching the region of interest, the first point attention gaze time bias score of.Statistical analysis was performed with SPSS 18.0 software. The count data of the experimental data were measured by chi-square test. The measured data were expressed by x ± S.Statistical analysis was carried out by using variance analysis and t test.P<0.05 was statistically significant.Results1.The study group fell 3 cases,27 cases completed the study, aged 18-45 years old, education of 9-15 years. The control group completed 30 experiments,18-45 years old,the education of 9-16 years.Compared with the control group, there was no significant difference in the gender, age and education years between the study group and the control group(P>0.05).2.The pre-treatment HAMD score,pre-treatment and post-treatment negative cognitive processing bias questionnaire scores in the study group were significantly higher than those in the control group(P<0.05);There was no significant difference in HAMD score between post-treatmen and control group(P>0.05).The scores of HAMD score and negative cognitive processing bias questionnaire in the study group were less than before treatment, the difference was statistically significant(P<0.01).3.The score of attentional bias in sad faces before and after treatment is higher in study group than in control group. The difference is statistically significant(P<0.05).The score of attentional bias in happy faces before and after treatment is lower in study group than in control group. The difference is statistically significant(P<0.05).The score of attentional bias in sad faces after treatment is lower than that before treatment(P<0.05).The score of attentional bias in happy faces after treatment is higher than that before treatment(P<0.05).4.The fraction of attentional bias in sad faces before and after treatment is higher in study group than in control group. The difference is statistically significant(P<0.05).The fraction of attentional bias in happy faces before and after treatment is lower in study group than in control group. The difference is statistically significant(P<0.05).The fraction of attentional bias in sad faces after treatment is lower than that before treatment(P<0.05).The fraction of attentional bias in happy faces after treatment is higher than that before treatment(P<0.05).5.Before the treatment, the score of Duration Before bias the study group reached the region of interest in sad faces is lower than that of the control group, and the difference is statistically significant(P<0.01);after treatment, the score of Duration Before bias the study group reached the region of interest in sad faces is not statistically significant compared with the control group(P>0.05).The score of Duration Before bias the study group reached the region of interest in happy faces is higher than that of the control group, and the difference is statistically significant(P<0.01);The score of Duration Before bias the study group reached the region of interest in sad faces after treatment is higher than that before treatment, and the difference is statistically significant(P<0.01).compared with the score of Duration Before bias the study group reached the region of interest in sad faces before treatment the difference is not statistically significant after treatment(P>0.05).6.Before the treatment, the the study group's score of First Fixation Duration in sad faces is higher than that of the control group, and the difference is statistically significant(P<0.01);after treatment, the the study group's score of First Fixation Duration in sad faces is not statistically significantcompared with the control group(P>0.05).Before treatment and after treatment, the study group's score of First Fixation Duration in happy faces is lower than that of the control group, and the difference is statistically significant(P<0.01).The score of First Fixation Duration in sad faces after treatment is lower than that before treatment, and the difference is statistically significant(P<0.01);compared with score of First Fixation Duration in happy faces before treatment the difference is statistically significant after treatment(P<0.05).Conclusion1.Patients with first depressive disorder have attentional bias of negative emotional faces, and the anti-depression treatment can improve the attention bias of negative emotional faces to a certain extent.2.Patients with first depressive disorder have attentional alert of negative emotional faces, and the anti-depression treatment can ease the attention alert of negative emotions to a certain extent.3.Patients with first depressive disorder have attentional maintain of negative emotional faces, and the anti-depression treatment appears to attentional avoid.
