Study On The Relationship Between No.6 And Its Subgroups Of Lymph Node Metastasis And No.14v Lymph Node Metastasis In Gastric Cancer

Purpose:To explore the correlation between No.6 and its subgroup No.6a,6v lymph node metastasis and the lymph node metastasis of No.14v group.Methods:Including 78 patients who underwent D2 + 14v radical gastrectomy.The lymph nodes of No.6 group were divided into two subgroups of group 6a and 6v with the right artery and vein of the gastric retina as reference.The detection rate of lymph node was increased by using nano-carbon tracer and the lymph node metastasis rate and metastasis of No.6 group and No.6a,6v and No.14v group were calculated..6a,6v two subgroups of lymph node transition,the relationship between clinical and pathological characteristics of gastric cancer were logistic regression analysis.Result:The lymph node metastasis rate in group 6 was 33.3%(26/78);The lymph node metastasis rate in subgroup 6a was 24.3%(19/78);The lymph node metastasis rate in subgroup 6v was 24.3%(19/78);The lymph node metastasis rate in group 14v was 33.3%(26/78);No.6a subgroup of lymph nodes and No.6v subgroups of lymph nodes were not statistically significant(P=0.774);The correlation between the lymph node metastasis rate of No.6 group,its No.6a,6v subgroup and the lymph node metastasis rate of No.14v group was statistically significant(P values were 0.001,0.022,0.013 respectively).The percentage of metastatic lymph nodes of No.6 group was 25.8%(80/310);The percentage of metastatic lymph nodes of No.6a subgroup was 25.7%(37/144);The percentage of metastatic lymph nodes of No.6v subgroup was 25.9%(43/166);The correlation between the percentage of metastatic lymph nodes of No.6a subgroup and No.6v subgroup was not statistically significant(P=0.926);N stage and PTNM stage was related to the lymph node metastasis of No.6 group(P values were 0.001,0.005 respectively);T stage,N stage and PTNM stage was associated with No.6a subgroup lymph node metastasis(P values were 0.049,0.001,0.005 respectively);N stage and PTNM stage was associated with No.6v Subgroup lymph node metastasis(P values were 0.001,0.007 respectively);N stage and PTNM stage was associated with No.14V lymph node metastasis(P values were 0.001,0.046 respectively);Logistic regression analysis showed that:N staging were associated with No.6 and its No.6a,6v subgroup lymph node metastasis(P values were 0.001,0.001,0.001 respectively);The lymph node metastasis of No.6a subgroup was independent risk factor of the lymph node metastasis of No.14v group(P=0.022).Conclusion:1.No.14v lymph node metastasis was related to No.6,No.6v subgroup lymph node metastasis,TNM stage and N stage were related to No.14v lymph node metastasis.2.No.6a subgroup lymph node metastasis is an independent risk factor for lymph node metastasis of No.14v.group;3.N stage and the No.6a and No.6v subgroup of lymph node metastasis are related to a certain extent,the bottom cleaning No.6a,No.6v subgroup of lymph nodes is helpful to improve the accuracy of N staging.It is important to predict the prognosis of patients with gastric cancer;For advanced gastric cancer with lymph node metastasis in No.6 group,especially the No.6a subgroup of lymph node metastasis in patients,suggest regular D2+No.14v radical surgery.
