Objective The research objects were patients with moderate and severe cancer pain who were accepted 21 days cycles of chemotherapy,by observing variation of pain and opioid medication in patients during pre-treatment,post-treatment and different time point of follow-up period,to evaluate the effect of analgesia by three ladder analgesic method combined with transcuataneous electrical Acupoint stimulation(TEAS);Through subjecting the quality of life of patients in different timeto evaluate the potential advantages of three ladder analgesic method combined with TEAS.Methods Based on randomized controlled study principles,setting up sample diagnosis criteria inclusion criteria,exclusion criteria and abscission criteria.From 278 patients with pain in Zhejiang Cancer Hospital,screening out of the 168 patients meetted to the standard of moderate and severe cancer pain,then randomly divided them into control group(simple three steps analgesic method)87 cases(abscissing 15 cases,it is actually 72 cases)and treatment group(three ladder analgesia method combined with TEAS)81 cases(abscissing 6 case,it is actually 75 cases)using the envelope method.Control group only accept three steps analgesic therapy;On the basis of three steps analgesic therapy,treatment group accept TEAS therapy in addition.TEAS(dilatational wave,2/100 Hz,current strength for patient tolerance degree)was applied bilaterally to HeGu and Wai Guan,ZuSanLi and SanYinJiao for 30 min,twice per day,5 days per week,stop 2 days,for 3 consecutive weeks.Throuth evaluating pain in patients with Brief Pain Inventory(Pre-treatment,post treatment)and recording patients equivalent dosage of morphine(Pre-treatment and post treatment,1 week follow-up and 2 weeks follow-up)to evaluate the pain situation of the two group.Evaluating patients quality of life by the Karnofsy function state scale(KPS).Results(1)Pain degree: Before treatment,there were no statistical differences in the most severe pain,slightest pain,average pain and the present pain score between control group and treatment group(P> 0.05).3 weeks after treatment,compared with pre-treatment,pain index scores were all decreased in two groups,And treatment group is more significant than control group(P<0.05).(2)The equivalent morphine consumption: The 24-hour equivalent dose of morphine in the treatment group at each time point was lower than the control group,but there was no significant difference between the two group.(3)The quality of life: Before treatment,KPS score had no difference between the two groups(P> 0.05);When post treatment,followed up for 1 week and 2 weeks,KPS score of both groups are increasing,and treatment group had more advantage than control group(P< 0.05).(4)The situation of safety and reception: There was no adverse reaction in the two groups during the treatment.The safety of evalution in both groups was geade 1;both groups had good reception and hadstatistical significance(P< 0.05).Conclusion Analgesic efficacy of three steps analgesic method combined with TEAS can effectively reduce moderate and severe cancer pain patients but it could not evidently reduce the dosage of opioids;Analgesic efficacy of three steps analgesic method combined with TEAS is safe and has no side effect,it can improve the quality of life of patients.
