Objective: In this study,regard the rats whose suffered L5 spinal nerve ligation(SNL)neuropathic pain as the research object,from the angle of behavioral testing and molecular biology tests analysis analgesic effect of massage on SNL rats.The molecular mechanism and changes of VGLUT2 in the transmission of peripheral pain information to explore the analgesic mechanism of massage.This study provides a safe,effective and simple method for clinical chronic neuropathic pain.It also provides a scientific basis and theoretical basis for further research on massage analgesia.Methods: 72 male SD rats were randomly divided into two groups.The first group was divided into normal group(n=16),sham group(n=16),and model group(n=40).SNL model was established by ligation of L5 spinal nerve in the model group,eight rats in each group were randomly selected for behavioral testing and draw materials after 7 days of modeling.In the second randomized group,32 rats in model group were randomly divided into model group(n=8),lentivirus injection group(n=8),sham massage group(n=8),and pivot meridian massage group(n=8).Rats in the chronic virus injection group were injected with a lentivirus particle that down-regulated VGLUT2 and injected only once;the rats in the fake massage group were gently stroked back for 18 minutes once a day for a total of 10 days;the rats in the pivot massage group adopt massage simulation apparatus to stimulate the gall bladder meridian Huan Tiao point,Feng Shi point and Yanglingquan point in turn,the stimulation method for the point-pressing manipulation,dial method and kneading manipulation,each method per point 1 minute,a total of 10 days intervention,behavioral testing and drawing materials after the intervention.The hyperalgesia of each group was observed by thermal pain threshold measurement and mechanical pain threshold measurement;the motor function of rats in each group was evaluated by sloping plate test;the expression of VGLUT2 protein in lumbar spinal cord enlargement segment was detected by immunohistochemistry and Western blot.Results: 1.Result of weight changes in rats:after 7 days of making model,compared the normal group and the sham operation group with the model group,the difference was statistically significant(p<0.05),but there were no significant differences between the normal group and the sham operation group(p>0.05).There were no significant differences between the normal group and sham operation group after 10 days of intervention(p>0.05).The differences between normal group,sham operation group and model group,chronic virus injection group,sham massage group,pivot meridian massage group were statistically significant(p<0.05).There were no significant difference in weight between the chronic virus injection group,sham massage group and pivot meridian massage group(p>0.05),and the difference was statistically significant(p<0.05)compared with the model group.2.Thermal pain threshold determination resumlts:There was no statistically significant difference between the normal group and the sham-operated group(p>0.05),after 7day of making model,the difference between the normal group,the sham operation group and the model group were statistically significant(p<0.05).After intervention for 10 days,there were no significant differences between the normal group,the sham operation group,chronic virus injection group and the pivot meridian massage group (p>0.05),and the difference was statistically significant by comparing the model group,but there were no significant between the model group and the sham massage group(p>0.05).3.Mechanical pain threshold determination results:There was no statistically significant difference between the normal group and the sham-operated group(p>0.05),after 7 days of making model,the difference between the normal group,the sham operation group and the model group were statistically significant(p<0.05).After intervention for 10 days,there were no significant differences between the normal group,the sham operation group,chronic virus injection group and the pivot meridian massage group(p>0.05),and the difference was statistically significant by comparing the model group,but there were no significant between the model group and the sham massage group(p>0.05).4.The result of inclined plate experiment showed that there were no significant differences between the sham operation group and the model group after 7 days of medaling(p>0.05),while the difference between the sham operation group,the medal group and the normal group were statistically significant(p<0.05).After intervention for 10 days,the experimental data of inclined plate showed no significant difference in the normal group,the sham operation group,the model group,chronic virus injection group,the sham massage group and the pivot meridian massage group(p>0.05).5.Immunohistochemical results showed that weakly positive(+-)accounted for 12.5% and positive(+)accounted for 87.5% in the normal group;positive(++)accounted for 62.5% and in the sham operation group,strongly positive(+++)accounted for 37.5%;strongly positive(+++)accounted for 100% in the model group after 7 days of medaling.After intervention for 10 days,positie(+)accounted for 100% in the normal group:positive(+)accounted for 87.5% and positive(++)accounted for12.5% in the sham operation group;strongly positive(+++)accounted for100% in the model group;weakly positive(+-)accounted for 75% and positive(+)accounted for 25% in the chronic virus injection group;positive(+++)accounted for 100% in the sham massage group;positive(+)accounted for 100% in the pivot meridian massage group.6.Western blot determination results:There were significant difference between the sham operation group and the normal group(p<0.05),after 7 days for medaling,the differences between the sham operation group and the model group were statistically significant(p<0.05),and the differences between the model group and the normal group were statistically significant(p<0.05).After intervention for 10 days,there were no significant differences between the normal group,the sham operation group,and the pivot meridian massage group(p>0.05),there were no significant differences between the model group and the sham massage group(p>0.05),there were significant differences in the normal group,the sham operation group and the pivot meridian massage group and chronic virus injection group(p<0.05),the differences between the normal group,the sham operation group,the pivot meridian massage group and the model group,the sham massage group were statistically significant(p<0.05).Conclusions: 1.SNL model does not affect the motor function of rats.Pivot massage can promote the recovery of thermal pain threshold and mechanical pain threshold in SNL rats,which means that the treatment of pivot massage can improve the sensory function of SNL rats.2.The injury pain of skin,muscle tissue,and nerve can all cause the VGLUT2 expression increased.By targeting the down-regulation of VGLUT2 expression in spinal cord neurons of rat,it can improve the pain appearance of rats and restore rats the thermal pain threshold and mechanical pain threshold to normal group level.3.Pivot massage can downregulate the expression of VGLUT2,and then regulate the release of glutamatic acid in the synaptic terminal neuroactive substances in the spinal cord,and inhibit the transmission of pain information.This may be one of the onset mechanisms of analgesia through massage.
