Objective:1?To observe the blood know luo jiu before and after treatment in patients with subcortical ischemic vascular dementia MMSE scale,HDS-R scale changes,evaluation of blood know luo moxibustion therapy in patients with subcortical ischemic vascular dementia clinical curative effect.2 ? Based on diffusion tensor imaging(DTI)technologies and fiber beam tracing method of spatial statistics(TBSS)on patients with subcortical ischemic vascular dementia and ultrastructure of cerebral white matter differences between healthy elderly ultrastructural changes and study the brain white matter and the correlation between the cognitive dysfunction.Method:1?Blood know luo the clinical curative effect of acupuncture treatment of subcortical ischemic vascular dementia research: according to diagnostic criteria in 25 SIVD patients,randomly divided into blood know luo moxibustion group and foundation treatment,blood know luo jiu group give blood know luo moxibustion treatment,foundation treatment group donnelly,pp qi oral,two groups of treatment for 12 weeks,before and after treatment respectively on two groups of patients with MMSE scale,HDS-R scale and clinical effectiveness evaluation,and the results are within the group before and after the contrast and comparison analysis between groups.2?SIVD patients with cerebral white matter diffusion tensor imaging of TBSS analysis: based on the test were included in the patient selection to meet the requirements of the experiment of 14 patients with SIVD,recruiting and 18 cases of age and gender matched healthy elderly as blank control group,routine mri scanner acquisition,T1,T2 weighted imaging-3 d BRAVO sequence and DTI sequence data.Using FSL software and TBSS method,two groups of data were compared and analyzed.Result:1?Evaluation results of MMSE scale: The MMSE score of the patients treated with stasis and moxibustion group was higher than that before treatment,with significant statistical difference(p<0.01).The MMSE score of the patients in the basic treatment group was higher than that before treatment,and the difference was statistically significant(p<0.05),and the difference in the post-treatment group was statistically significant(p<0.05).2?HDS-R scale assessment results: After treatment,HDS-R score was higher than that before treatment,with significant statistical difference(p<0.01).After treatment,the HDS-R score in the basic treatment group was higher than that before treatment,and the difference was statistically significant(p<0.05),and the difference between the treatment group was statistically significant(p<0.05).3?MMSE curative effect evaluation of the results: the two groups before treatment compared with the treatment of blood know luo jiu group total effective rate was92.85%,foundation treatment group total effective rate was 81.81%,compared two groups of clinical curative effect,with statistical difference(p < 0.05);4?Analysis results of the diffusion tensor imaging of white matter in the brain:4.1 Compared with healthy controls,SIVD patients in a wide range of white matter fiber tracts anisotropic index(FA)showed lower,over SIVD patients in bilateral corpus callosum(mostly on the knees and body),bilateral cingulate central beam,on the left side of the pillow beam(mainly on the left side of the internal capsule forelimb),on the left side of the longitudinal fasciculus,outside on the left side of the capsule(including the frontal pillow beam,beam hook),bilateral frontal pillow under the beam,the thalamus after radiation and hook,double beam lateral prefrontal subcortical white matter such as FA values decreased significantly(P < 0.005,FWE correction),on the left side of the white matter changes significantly more than the right side;4.2 Compared with healthy controls,SIVD patients in a wide range of white matterfiber tracts in the average diffusion rate(mean diffusivity,MD)are showing up,SIVD patients in the corpus callosum knee and body,under the bilateral frontal pillow on the beam,internal capsule,longitudinal fasciculus,bilateral cingulate such as beam of MD values significantly higher(P < 0.005,FWE correction);4.3 Compared with healthy controls,SIVD patients in a wide range of white matter fiber tracts in the radial diffusion coefficient(radial diffusivity,RD)are showing up,RD value increase SIVD disease area and FA similar figure mainly includes the corpus callosum,the longitudinal fasciculus,internal capsule,strap on the right side,under the pillow bouquets,dome,the left hook,thalamus after radiation beam,but the body and corpus callosum under pressure,the left cingulate gyrus,frontal pillow beam FA values to reduce regional than RD value increased(P < 0.005,FWE correction);Conclusion:1?It is an effective method for clinical treatment of SIVD,which can improve patients' learning and memory ability and intelligence level,and can improve the quality of life of elderly patients.2?DTI technology on microstructure changes of the brain show more sensitivity and specificity,can be sensitive to detect SIVD patients with cerebral white matter fiber tracts widespread damage.3?Cerebral white matter fiber tracts connectivity and the integrity of the interrupt and its cognitive dysfunction may be related...
