Objective:1.Through observing the effect of Gandoudihuang decoction on the UWDRS,Frenchay articulation disorder assessment form and TCM Syndrome scores of brain WD patients,the effect of Gandoudihuang decoction on dysarthria was understood,which provided a reference for clinical treatment of this type of patients.2.Through the statistical analysis of brain WD dysarthria patients with lesions of MRI lesions in the brain to observe the correlation between lesion location and dysarthria in such patients.3.By observing the effect of Gandoudihuang decoction on the urine copper of brain type WD patients,the effect of copper excretion was understood.Methods:1.Grouping:According to inclusion criteria and exclusion criteria,90 patients with brain type WD accompanied by dysarthria were divided into three groups: A,B and C,and the patients in group A were treated with Gandoudihuang decoction +acupuncture + DMPS Group,group B acupuncture + DMPS group,group C DMPS treatment group,each group 30 cases.Totally 82 patients with brain type WD dysarthria were collected from 26 patients in group A,27 patients in group B and 29 patients in group C.2.Indicators and curative effect: The treatment cycle is 4 treatment course,urine copper taken before treatment and the course of treatment.Recording lesions in patients with brain magnetic resonance lesions,Before and after treatment,the scores of the Unified WD Rating Scale(UWDRS),Frenchay Acoustic Dysphoria Scale,TCM Syndrome Score Table were observed.And observe the safety indicators before and after treatment changes.Results:(1)UWDRS score comparison:There was a significant difference before and after treatment(P<0.01)in the three groups.There was a significant difference between group A and group B after treatment(P<0.05).There was a significant difference between group A and group C after treatment(P<0.01).(2)The scores of the three groups in the Frenchay assessment table were significantly different before and aftertreatment(P<0.01).Compared between groups,there was a significant difference between group A and group B after treatment(P<0.05).There was significant difference between group A and group C after treatment compared with group B and C group(P<0.01).(3)The comparison of the urine copper: each of the three groups of patients before treatment and treatment of 24 h urinary copper content were significantly higher(P<0.01);group A and group C second courses and fourth courses of urine copper treatment differences(P<0.05);group A and group B urinary copper second course treatment significantly(P<0.01),the fourth course of treatment the treatment of urinary copper had significant difference(P<0.05).here was a significant difference in urinary copper content between the four groups(P<0.01)during the four treatment courses after treatment in the three groups.There was significant difference between group A and group B and group C(P<0.01);There was no significant difference between group B and group C.(4)Comparison of curative effect:The significant efficiency was compared: the significant efficiency of A group was 80.7%,group B was 62.9%,and that of group C was 44.8%.The difference between the three groups was significantly different(P<0.01),and the group A was higher than the B group(P<0.05),and the significant difference between the A group and the C group was significantly different(P<0.01).There was no significant difference in the efficiency of the B group compared with the C group.The total effective rate was compared: group A was 96.1%,group B was 85.1%,and group C was 68.9%.The total effective rate of the three groups was significantly different(P<0.05).(5)TCM syndrome score comparison: after three groups of treatment,TCM syndrome scores were obviously improved(P<0.01).After treatment in group A,compared with group B and group C,there was a significant difference(P<0.05),but there was no significant difference between group B and group C(P>0.05).(6)Brain MRI: The most common lesion of brain MRI in patients with brain type WD with dysarthria is basal ganglia,the common lesions in this kind of patients are bean nucleus,thalamus,middle brain,bridge brain,caudate nucleus,etc.(7)Age distribution: Age analysis of brain typeWD patients with dysarthria showed that the highest number was between 15-20 years old,followed by 25-30 years old,and 73.17% between 15-30 years old,while15-40 yearold accounted for up to 95.12%.(8)Adverse reactions: There was no difference in the incidence of adverse reactions between the three groups(P>0.05).Conclusion:(1)Gandoudihuang decoction can effectively improve the dysarthria symptoms of brain type WD patients,combined acupuncture treatment is better;(2)The common lesions of brain MRI in patients with brain type WD with dysarthria are in turn the lenticular nucleus,thalamus,mesencephalic pontine,caudate nucleus,etc.(3)Gandoudihuang decoction can significantly improve the urinary copper excretion of patients.
