Objective:To investigate the effect of extended hepatectomy on early liver function and to explore its relationship with pentose phosphate pathway.Methods:One hundred and five Sprague–Dawley rats were divided into three groups randomly.Standard partial hepatectomy group:68%of liver resection(mid lobe+left lobe),extended hepatectomy group:86%of liver resection(mid lobe+left lobe+caudate lobe+right lower lobe),glucose intervention group:86%of liver resection and drink 20%glucose after operation.Fifteen SD rats were used to postoperative survival analysis,and the rest of rats were sacrificed at indicate time points.Liver function index——AST and ALT were determined using Biochemical autoanalyzer.The proliferation index Ki-67 was observed by immunohistochemistry.The mRNA expression of G6PD was detected by real-time fluorescent quantitative PCR.The protein expression of G6PD was detected by western blot.The ratio of NADP~+/NADPH was detected by NADP~+/NADPH kit.Result:Compared with standard partial hepatectomy group and extended hepatectomy group——the 5-day survival rate of the standard partial hepatectomy group was 100%,and extended hepatectomy group was46.7%.At 6h and 48h after operation,the levels of AST and ALT in extended hepatectomy group were significantly increased(p<0.05).At 48h after operation,the Ki-67 expression were significantly decreased in extended hepatectomy group.At 48h after operation,the ratio of NADP~+/NADPH was significantly increased(p<0.05).Within 24h after surgery,G6PD mRNA level is on the decline,and at 24h postoperatively,the degree of decline was more pronounced in extended hepatectomy group(p<0.05).And at 48h,the expression of G6PD mRNA was increased in extended hepatectomy group,but was significantly lower than that in standard partial hepatectomy group(p<0.05).At 48h after operation,the expression of G6PD protein was significantly decreased in extended hepatectomy group(p<0.05).After rats with extended hepatectomy were treated with 20%glucose,the levels of AST and ALT were significantly decreased at 48h postoperatively(p<0.05).At 48h after operation,the Ki-67 expression increased significantly and the NADP~+/NADPH ratio decreased significantly(p<0.05).The expression of G6PD mRNA was significantly increased at 24h and 48h postoperatively(p<0.05),and the expression of G6PD protein was also significantly increased at 48h postoperatively(p<0.05).Conclusion:After extended hepatectomy,the liver function was severely impaired and liver regeneration was weakened in early stage.The pentose phosphate pathway was weakened after extended hepatectomy.The liver function was severely impaired and liver regeneration was weakened may be related to postoperative decline in pentose phosphate pathway after extended hepatectomy.
