Objective: To assess the left atrial function in patients with iron deficiency anemia(IDA)by two-dimensional speckle tracking imaging(2D-STI).Methods: We selected 65 IDA patients,they were diagnosed in the Department of Hematology,Gynecology and Digestive Department of our hospital from November 2014 to June 2016 to be experimental group,male:female=1:4.5,they were divided into group B(Hb>90 g/L,age 25~65 years,mean 48.8±14.1 years,duration 7 month~15 years,mean 6.5±3.3 years)and group C(Hb60~90g/L,age 22~62 years,mean 47.6±16.6 years,duration 8 month~14 years,mean 6.2±3.6 years)according to the degree of hemoglobin.The entry criteria for IDA inclusion: Male hemoglobin(Hb)<13 g/dL,female Hb<12 g/dL,pregnant women Hb<11 g/dL.MCV<80 fl,MCH<27 pg,MCHC<32%.And there was the basis of iron deficiency.38 patients with IDA were caused by gynecological disease such as uterine leiomyoma,adenomyosis and increased menstrual flow.18 patients with IDA were caused by digestive system disease such as subtotal gastrectomy.Another 9 cases with IDA were caused by unknown reasons.Exclusion of other heart disease,such as congenital factors,diabetes,coronary heart disease,hyperthyroidism,hypertension and so on.Remove of patients who received chemotherapy and radiotherapy after major surgery of gastrointestinal malignant tumors.A family history and other chronic diseases were also discharged.Another 30 people who took routine examination in our hospital from November 2014 to June 2016 were also selected as control group(age25~65 years,mean 48.8±14.1years,male:female=1:5).Their physical examination were all normal.Exclusion of family history and other chronic diseases.An ultrasound system(Vivid E9,GE Medical Health,USA)and an M5S-D probe(1.5-4.5 MHz)were used for the study.The image frame rate ?40 frame/s.We used Echo PAC analysis software to analyze the strain rate of left atrial wall.All patients were being informed and consented to participate in this research.The parameters of measurement included:1.Conventional parameters of groups A,B and C included left atrial end-systolic diameter(LAD),peak E and A of mitralis(E,A),E/A,end-diastolic thickness of ventricular septum(IVSTd),end-diastolic thickness of LV posterior wall(PWTd),left ventricular end-diastolic dimension(LVDd),left atrial minimum volume(LAVmin),left atrial pre-atrial contraction volume(LAVp),left atrial maximum volume(LAVmax),then left atrial active ejection fraction(LAAEF)and left atrial passive ejection fraction(LAPEF)were calculated.2.2D-STI parameters of groups A,B and C included the ventricle systolic strain rate(SRs),early diastolic strain rate(SRe)and late diastolic strain rate(SRa),these date were collected and then the average strain rate(mSRs,mSRe and mSRa)were calculated.Results:1.Conventional parameters: IVSTd,PWTd,LVDd,LAD,A,E,E/A,LAVmax,LAVmin,LAVp,LAAEF and LAPEF were no obvious differences between group B and group A(all P>0.05).The LAD,A,LAVmax,LAVmin,LAVp,LAAEF of group C were higher than those of groups A and B(all P<0.05).The E,E/A,LAPEF of group C were decreased than those of groups A and B(all P<0.05).2.2D-STI parameters: mSRs,mSRe and mSRa were no obvious differences among groups A,B and C(all P>0.05).The mSRa of group C was higher than groups A and B(P<0.05).The mSRs,mSRe of group C were decreased than those of groups A and B(all P<0.05).Conclusion:The strain rate of the left atrial varies with the degree of iron deficiency anemia,2D-STI could evaluate the left atrial function in patients with IDA.
