Background and Objective Inflammatory bowel disease(IBD)is a chronic and recurrent intestinal inflammatory disease,mainly including Crohn disease(CD)and ulcerative disease(UC).The diagnosis of IBD is mainly depending on clinical manifestation,gastrointestinal endoscopes,histopathological biopsy and follow-up study.There is no "golden standard" in the diagnosis of IBD.Then searching biomarkers of IBD is an important subject in this field.microRNA(miRNA)is a small and single non-coding RNA,18-23 nt long,which is stable outside cells and can be used as the biomarkers of diseases.Then the objective of my research is to exploring fecal miRNA in IBD as the biomarkers.Method In this study,we collected stool samples from 23 patients with UC and 30 patients with CD and 30 healthy people.Then we extracted miRNA from 200-300mg stool sample,and mixed 9 extract liquor randomly per group,which was to sequence the human genome of miRNA.According to the result,we selected three miRNAs(miR-143-3p,miR-148a-3p and miR-1246)to do absolute quantitative PCR.Then the Mann-Whitney U test was used to statistical analysis.Result The research finds that:1)The expression of fecal miR-148a-3p and miR-1246 in clinical samples of IBD patients is significantly higher than stool samples in normal controls from healthy people(p<0.05).And the expression of fecal miR-148a-3p and miR-1246 in UC group and CD group are both higher than normal group(p<0.05).But the expression of fecal miR-143-3p in IBD group has no significant statistical difference with normal group(p>0.05).2)Area under ROC curve of miR-148a-3p expression for IBD is 0.796.When the cutoff value is 0.46 copies/ug,the sensitivity of miR-148a-3p is 72.2%and the specificity is 80.6%;Area under ROC curve of miR-1246 expression for IBD is 0.815.When the cutoff value is 8.11 copies/ng,the sensitivity of miR-148a-3p is 81.1%and the specificity is 80.6%.3)The expression of fecal miR-1246 in the IBD patients with active periods is significantly higher than IBD patients with paracmasis(p<0.05).Conclusion The expression of fecal miR-148a-3p and miR-1246 in IBD patients are both significantly higher than healthy people.And they can help the diagnosis of IBD.At the same time,the expression of fecal miR-1246 in the IBD patients with active periods is significantly higher than IBD patients with paracmasis,which may be used to evaluate the activity of IBD.
