Obejectives(1)To investigate the relationship between pain beliefs,self-efficacy and sleep quality in patients with total knee arthroplasty(TKA).(2)To analyze the factors influencing sleep quality in total knee arthroplasty(TKA)patients.(3)To explore the correlation between pain beliefs,self-efficacy and sleep quality in patients undergoing the total knee arthroplasty.MethodsIn this descriptive study,we selected 216 patients who underwent knee arthroplasty in three tertiary orthopedic hospitals by use random cluster extraction method from February to December in 2016.The general information questionnaires,pain and narcotic analgesic beliefs scale,self-efficacy scale and Pittsburgh Sleep Quality Index(PSQI)were investigated.All data were recorded with Epidata3.1.And analyzed with SPSS22.0.Statistical methods included descriptive statistical analysis,Pearson correlation analysis and multiple stepwise regression analysis.Results(1)216 patients underwent total knee arthroplasty,the sleep quality score was(7.63±4.54),the subjective sleep quality score was(1.26±0.89),the fall asleep time score was(1.25±0.92),the sleep time score was(0.72±0.95),the sleep efficiency score was(1.27±1.24),Sleep disorder score(1.53±0.62),hypnotic drug score(0.29±0.51),daytime dysfunction score(1.31±0.93).(2)In the 216 total knee arthroplasty patients,The total pain belief score was(21.72±7.90),the patience pain score was(12.36±4.20),the side effect of analgesics was(10.36±3.96).Self-efficacy score was(6.02±2.54),the self-efficacy dimension of symptom management was(5.96±2.60),and the self-efficacy dimension of disease common management was(6.13±2.50).(3)The results of univariate analysis in the 216 total knee arthroplasty patients showed that age,gender,degree of educational,household per capita monthly income,family structure,surgical site,duration of illness,pain degree and degree of dysfunction were statistically significant(P<0.05)in patients with total knee arthroplasty,P values are0.021,0.016,0.038,0.025,0.005,0.001,0.002,0.007,0.043.(4)Multivariate analysis of sleep quality in 216 patients with total knee arthroplasty:gender,pain degree,surgical site,degree of educational are entered the regression model,and the fitted linear regression equation was statistically significant(F=10.315,P=0.000).The complex correlation coefficient of the equation R=0.766,the decision coefficient R~2=0.417.(5)By pearson correlation analysis:The total score of sleep quality was negatively correlated with patience pain,analgesic side effects,and pain scores,r values are-0.690,-0.695,-0.698.The total score of sleep quality was positively correlated with self-efficacy score,symptom mangement self-efficacy,disease commom management self-efficacy,r values are0.717,0.684,0.725.Conclusion(1)The total knee arthroplasty patients had a poor sleep quality.(2)Gender,degree of educational,surgical site,pain degree are factors on the sleep quality in total knee arthroplasty patients.(3)There was a negative correlation between pain beliefs and sleep quality in patients with total knee arthroplasty,and self-efficacy was positively correlated with sleep quality.The higher the negative pain beliefs,the higher the self-efficacy,the worse the quality of sleep in patients with total knee arthroplasty.
