With China's continuously increased requirements on hospitals' management,hospitals in China have made uninterrupted efforts to improve their surveillance on the quality of medical care.It has now become an inevitable trend for China to use modern information tools to strengthen hospitals' monitoring.However,most of hospitals' information constructions are focused on business process management and payment method,few of them involve in the surveillance of hospitals' medical quality.As for whether the doctors' diagnosis and treatment are conforming to clinical guidelines,whether there exists such phenomena as over-treatment or under-treatment,whether the drug use is safe and whether the diagnosis and treatment of patients with health insurance are conforming to the rules of healthcare policy,there is still not too much experience to draw from to deal with the above-mentioned problems.This article is mainly expounding how to use big data technology and clinical knowledge bank to establish an intelligent monitoring system in a hospital.By collecting data from HIS(Hospital Information System)which is plugged in the hospital,and then analyze and calculate them to find out the violations in the hospital.The paper researches how to analyze and make intelligent supervision of medical big data by a system and other aspects in order to solve some practical problems in medical governance and monitoring in hospitals.Major research contents are as follows:First,The paper researches establishment and realization manner of a medical and pharmaceutical clinic knowledge base.The key of knowledge base intellectualization lies in a thinking "brain" of the system.The paper establishes,manages and applies a clinic knowledge base;analyzes structural structure,business logics and realization method according to relations between diseases and medicines,relations among diseases,inspection items and treatment items,taboo combinations between medicines,indications of medicines,routines of diagnosis and treatment of diseases and other aspects;and sets up a complicated logic relationship.A non-structured and configurable and flexibly-applicable configuration management subsystem is researched and applied for storage and management of clinic knowledge.The clinic knowledge base is the professional foundation of medical big data analysis and also the professional basis for intelligent supervision.Second,The paper analyzes,designs and applies "rules" in intelligent supervision.According to rules and policies of medical management,the research designs and establishes an analysis and supervision rule engine,manages policies and rules of text description in a logical manner and solidifies them into the rule engine.By designing the architecture,the paper researches the deployment mode of distributed computation and realizes it in actual work.In this way,investment in IT equipment is saved,while efficiency and expandability of computation are also increased greatly.Third,The paper researches how to monitor and find doctors' medical behaviors which violate clinic routines among numerous medical data through the big data technology.Medical behaviors can be analyzed by sample data,local data and one-sided data in big data,so that medical insurance bureaus can get rid of the pursuit for causality.Analysis methods and system are designed by the thought of big data.Discovery of outlier data helps users find suspicious data and carry out efficient management.Besides researching concept of big data and application ideas,the paper also indeed practices concept of big data and ideas of application in actual work.Through design,analysis,system construction completion and test research,the paper concludes that the analysis and intelligent supervision system of medical big data,which is established based on knowledge base and examination rules,is feasible and very effective.After smooth completion of this project,it is feasible to further research how to generalize the project into supervision of regional medical treatment;supervise wide-area medical behavior norms;further explore data of larger amount;and promote policy analysis and execution of regional medical policies.All of these practices will bring very active practical significance.
