Objective:To observe leukoaraiosis imaging changes in infarction patients caused by arterothrombosis,analyze the relation of cervical atherosclerotic plaque,intracranial artery stenosis,extracranial artery stenosis with leukoaraiosis which caused by arterothrombosis,discuss mechanism which caused by arterothrombosis.The result may provide some theoretical basis for prevention and treatment of Leukoaraiosis.Methods:We recruited acute arterothrombosis infarction patients and small artery disease infarction patients who suffered infarction within a week.Meanwhile we recruited control group who were diagnosed benign location vertigo and idiopathic facioplegia.Leukoaraiosis changes on MRI were rated by the method of Fazekas et al and divided into three types:subcortical,periventricle and mixed.Leukoaraiosis types and symmetry were compared among control,AT and SAD groups.Arterial intima-media thickness(IMT),the type of carotid artery plaques,bulbus caroticus plaques and internal carotid plaques,the number of plaques,the location of plaques,the maximum size of plaque,carotid artery stenosis,intracranial artery stenosis,extracranial artery stenosis,mixted artery stenosis were compared between two sides which leukoaraiosis was asymmetry in arterothrombosis infarction patients.Results:1.We collected 59 arterothrombosis infarction patients,99 small artery disease infarction patients,50 controlled patients.The age and proportion of coronary heart disease in AT patients was lower than that of in SAD patients P value was 0.032,0.002 respectively),the proportion of hypertension history in former was higher than that of control patients(P value was 0.024);The proportion of age,smoking history and hyperlipemia history in SAD patients was higher than that of in control patients(P value was 0.002,0.029,0.019 respectively).2.In 59 AT patients,17 patients were divided into subcortical type,39 patients mixed type and 1 patient periventricular type.In 99 small artery disease infarction patients,1 patient was divided into subcortical type,28 patients peripheral type and 66 patients mixed type.The proportion of subcortical type in AT group(17/59)was much more than that in SAD group(1/99)(P value was 0.000),the difference was statistically significance.The proportion of periventricular type in SAD group(28/99)was much more than that in AT group(1/59)(P value was 0.000),the difference was statistically significance.There was no difference in the proportion of mixed type between AT group(39/59)and SAD infarction group(66/99)(P value was 0.942).LA ratio in AT group(57/59)and SAD group(85/99)was higher than that of in control group(26/50)(P value was 0.000).3.There were 35 asymmetry LA in 59 AT patients;There were 13 asymmetry LA in 99 SAD patients;There were 5 asymmetry LA in 50 control patients.Asymmetry LA was compared between AT group and SAD group,the difference was statistically significance(P value was 0.000);Asymmetry LA was compared between AT group and control group,the difference was statistically significance(P value was 0.000);Asymmetry LA was compared between SAD group and control group,the difference was no statistically significance(P value was 0.058).4.In asymmetry LA of the AT group,the proportion of soft plaque,mixed plaque,carotid artery plaque,internal carotid plaque in more serious side was higher than that of in milder side(P value was 0.008,0.000,0.034,0.007 respectively);While the proportion of no plaque,hard plaque,number of plaque,bulbus caroticus plaque,the maximum side of plaque,IMT was compared between more serious side and milder side in asymmetry LA and the difference was no significance(P value was 0.707,0.131,0.806,0.131,0.550,0.905 respectively).5.There were 35 asymmetry LA patients in the AT group,19 patients were diagnosised intracranial artery stenosis and 10 patients were diagnosised extracranial artery stenosis in seriousness,9 patients were diagnosised intracranial artery stenosis and 2 patients were diagnosised extracranial artery stenosis in mildness,the difference was statistically significance(P value was 0.015,0.030 respectively);6 patients were diagnosised mixted artery stenosis in more serious side,1 patients was diagnosised mixted artery stenosis in milder side,the difference was no statistically significance(P value was 0.111).Conclusions:1.Leukoaraiosis ratio in AT group and SAD group was higher than that of in control group.LA in AT patients mainly presented subcortical type,and the changes were asymmetric;LA in SAD patients mainly presented periventricular type,and the changes were symmetric.2.The proportion of soft plaque,mixed plaque,carotid artery plaque,internal carotid plaque in more serious side was higher than those in milder side in AT group with asymmetric LA.3.The proportion of intracranial artery stenosis and extracranial artery stenosis in more serious side was higher than those in milder side in AT group with asymmetric LA.
