Objective:1.The first objective of the research was to detect serum microRNA of IBS-D patients and healthy controls,Screen differentially expressed microRNA pedigree between the two groups for accurate diagnosis and treatment in the future.2.The second objective of the research was to study the differently expressed microRNAs before and after acupuncture treatment to find the effect of acupuncture on microRNAs.And to interpret the mechanism of acupuncture therapy on IBS-D.Methods:1.This study intake IBS-D patients and healthy controls,patients and healthy control was taken a certain amount of blood,the serum was separated,analyze the microRNA difference and differentially expressed in multiples between IBS-D patients and healthy controls by cDNA microarray analysis.2.IBS-D patients enrolled to soothing liver and strengthening spleen acupuncture treatment three times a week,for 6 weeks,a total of 18 times.Assess the treatment effect by IBS-SSS scale evaluation before and after the treatment.Take patients' blood samples again after acupuncture treatment,microRNAs in isolated serum samples before and after treatment were detected by real-time quantitative RT-PCR,on the one hand to verify the results of microarray gene,on the other hand analyze the difference of microRNA before and after treatment.Results:1.23 IBS-D patients and 19 healthy controls were enrolled without significant difference about age and gender between two groups.2.15 significant differently expressed microRNAs were detected by gene microarray test between 2 groups,namely miR-1305,miR-575,miR-149,miR-190,miR-135a,miR-210,miR-148a,miR-151-3p,miR-22,miR-483-3p,miR-194,miR-887,miR-1,miR-127-5p,miR-892b,of which miR-1305.miR-575.miR-149.miR-190.miR-135a?miR-210?miR-148a and miR-151-3p were significantly higher expressed in the IBS-D group than in the control group(twice or more),and miR-22.miR-483-3p?miR-194?miR-887?miR-l?miR-127-5p and miR-892b were significantly higher expressed in the control group than in the IBS-D group(twice or more)3.By acupuncture intervention,symptoms of IBS-D patients was significantly improved compared with the previous,IBS-SSS ratings before acupuncture treatment was241.52 ±67.17,while after acupuncture intervention was 73.70 ± 45.98(P<0.01),effective rate 91.30%.4.Real-time quantitative PCR study comparing serum miRNA before and after acupuncture treatment in patients with IBS-D,we found that miR-210 and miR-148a in the serum of patients after treatment were significantly lower than that of before treatment(P<0.05);while miR-22 and miR-194 significantly increased(P<0.05).Conclusion:1.Part serum miRNA between IBS-D patients and normal group was obvious abnormal,which indicate that a change in the concentration of the microRNA leads to target gene expression changes,resulting in pathological mechanism and symptoms occurred in IBS-D patients.2.Acupuncture treatment can significantly improve patients' symptoms.3.Acupuncture possibly through regulating the expression of miRNA in vivo IBS-D patients,indirect control of the expression of a downstream protein molecules,which play a role in improving the symptoms.Related studies show that specific binding mechanism may be by regulating acupuncture acupuncture-miRNA-immune regulation,acupuncture-miRNA-nervous system,acupuncture-miRNA-cell protection,acupuncture-miRNA-tumor suppression pathway which play a role in the treatment of IBS-D.
