ObjectiveTo investigate the correlation between the MRI morphology scoring of cartilage injury and the changes of T2 value in knee osteoarthritis(OA)and clinical manifestations.Materials and MethodsSixty-five knee osteoarthritis patients who had been diagnosed according to Osteoarthritis diagnosis and treatment guideline(Edition 2007)issued by Chinese Osteoarthritic Association,enrolled from March 2015 to October 2015,were grouped as the OA group.Ten age-matched healthy volunteers were enrolled as the control group.All research participants should be graded according to the Western Ontario and Mc Master University Osteoarthritis Index(WOMAC)and sign informed content before knee MRI examination.Knees of all research participants were imaged with MRI scanning using Philips Achieva 3.0T TX magnetic resonance imaging machine with an 8-channel knee coil on sagittal PDW-SPAIR,sagittal T1WI-a TSE,sagittal T2WI-TSE,coronal PDW-SPAIR,sagittal 3D-WATSc and sagittal T2.All images were sent to Philips Extended MR Work Space 2.6.3.5 to be further processed..Multiple Planar Reconstruction(MPR)was used to reconstruct 3D-WATSc sequence chart of sagittal view.A knee was divided into 5 subregions(the anterior,central,and posterior tibia,and the central and posterior femur)according to Whole-Organ Magnetic Resonance Imaging Score(WORMS).Cartilage morphology of these five subregions were evaluated and scored on a 0-6 scale using the WORMS.Patients in the group OA were further divided intogroup OA1(WORMS=0~1),group OA2(WORMS=2~3)and group OA3(WORMS=4~5).Sagittal T2 mapping would automatically create a T2 grey-scale chart after being processed.T2 values of all regions were manually measured 3 times and the average would be calculated and used.All data were analyzed by SPSS 19.0.Measurement data was expressed as (?) ±s,and all tests were conducted by two-sided test with 95% reliability and significance level 0.05.The chi-square(c2)test(count data)and t test(measurement data)were used to analyze the general data between group OA and the control group.Pearson's correlation coefficient was performed between T2 values of each sub region and WORMS and the correlation between T2 values/WORMS of group OA and WOMAC scores.The differences of T2 values among control group,group OA1,group OA2 and group OA3 were compared by one-way ANOVA SNK method.ROC curve was used to analyze the most significant T2 threshold value in subregions where differences of data were identified between the control group and group OA.Results1.The average age of sixty-five patients was(50.1±9.3)years old,19 males(29%)and 46 females(71%),27 right knees(41.5%)and 38 left knees(48.5%).The average age of ten healthy volunteers were(51.8±6.3)years old,3 males(30%)and 7 females(70%),4 right knees(40%)and 6 left knees(60%).There were no statistical difference in age,sex and the number of knees between the two groups(P>0.05).2.T2 values were positively correlated with WORMS scores in group OA(r=0.657-0.854,P<0.05),except for data in subregion of the anterior femoral.3.Except for data in the subregion of anterior femoral.,T2 values were not all equal in each group.T2 values of each subregion gradually increased with the grade level increasing.Apart from the T2 values in area of anterior tibia of OA1 group and the control group,and those in patella area of OA1 and OA2 groups had no significant differences,while the rest of the subregions had statistical differences between the two groups.4.T2 values in the sub-region of patella,posterior femoral and posterior tibiabetween control group and OA1 had statistically significant differences.The sensitivity of early knee OA using T2 mapping were 78.3%,68.2% and 54.8%,respectively,and the specificity were 100%,80% and 70%,respectively.5.T2 values of OA group had a positive correlation with WOMAC total scores,pain scores,stiffness scores and activity scores,with the correlation coefficient of0.501,0.384,0.357 and 0.512,respectively(P<0.05).WORMS scores of OA group was positively correlated with WOMAC total scores,pain scores,stiffness scores and activity socres,with the correlation coefficient of 0.806,0.690,0.493 and 0.817,respectively(P<0.05).Conclusions1.WORMS as MRI semi-quantitative evaluation index could reflect the morphological changes of cartilage of OA knees and reflect clinical manifestations of patients with knee OA to some extent.2.T2 values as MRI quantitative evaluation index could differentiate the severity of cartilage injury of OA knees,discover the cartilage lesions before morphological changes,and explain the clinical manifestations.
