Objective:In patients with stable angina, doing two phases treadmill exercise test respectively before and after taking trimetazidine, through the analysis of the parameters of a two stage records, research cardiac cells protection mechanism of trimetazidine. Methods:For 60 patients who conformed the inclusion criteria with two stages treadmill exercise test.In the first stage, after a washout period of 7-day without cardiovascular medications or antihyperglycemic drugs, all patients underwent the treadmill exercise test(T1).In the second stage, received only 20 mg thrid daily of trimetazidine for 7days,all patients went the treadmill exercise test(T2)again.Ischemic pretreatment was demonstrated with the parameters of the plate motion test.Compared with parameters of twice treadmill exercise test(T1,T2),including the time to 1.0 mm ST-segment deviation(T-1.0 mm)during the test, the maximum of ST-segment depression, the rate pressure product(RPP),to evaluate the role of the drug on myocardial protection mechanism. Results:In the first stage of all the 60 patients included,51 were tested positive for treadmill exercise test(horizontal or downsloping ST-segment depressions to 1.0mm),induced myocardial ischemia; In the second stage, after 1 week of trimetazidine,40 patients(78%)by demonstating significantly improvement in time to reach 1.0 mm ST deviation,a maximum of ST segment down by 0.19±0.09 mV of T1 decreased to 0.11± 0.06 mV of T2(P ﹤ 0.05),but the result of rate pressure product were not statistically significant. Conclusion:In this study, trimetazidine is beneficial to the myocardial cell protection mechanism in patients with stable angina pectoris, and does not reduce cardiac rate-pressure product or coronary blood flow.
