Clinical Study On The Effect On The Size Of Renal Pelvis With Artificial Hydronephrosis Of Pre-indwelling Double-J-tube During Percutaneous Nephrolithotomy

Objective:To explore the effect of hydronephrosis of renal pelvis with pre-indwelling double-J-Tube,and the effect of continuous reflux perfusion of bladder during percutaneous nephrolithotomy.Methods:80 patients with renal stone in our hospital from November 2016 to December 2017 were divided into 4 groups according to the degree of hydronephrosis: 12 cases with no hydronephrosis,31 cases with mild hydronephrosis,29 cases with moderate hydronephrosis and 8 cases severe hydronephrosis group.Respectively,the double J tube was used to manufacture artificial hydronephrosis,the change of hydronephrosis of renal pelvis before and after artificial hydronephrosis in each group were observed.Results:79 in 80 patients were successfully punctured in 2 needles,in 1 cases of antler type calculus,the one-stage operation puncture failed,and the second PCNL was successful.In no hydronephrosis group,mild hydronephrosis group and middle hydronephrosis group,the change of the pyelic separation before and after the artificial hydronephrosis were statistically significant(p <0.05),but there was no statistical significance in the change of pyelic separation before and after the artificial hydronephrosis in the severe hydronephrosis group(p >0.05).The obvious dilatation changes were observed after the artificial hydronephrosis in the target renal calyx(p<0.01).There was no significant hydronephrosis change in target calyx in 4 groups before and after puncture(p >0.05).The average operation time was 86.9min,the average length of hospitalization after operation was 6.9days,all patients did not have serious complication,the one-stage stone clearance rate was 93.7%.Conclusion:Pre-indwelling double J tube during percutaneous nephrolithotomy could form stable hydronephrosis,improve the efficiency and safety of PCNL,continuous reflux perfusion of bladder can help to expel stone.This surgical method is of a variety of advantages,worthy of clinical promotion.
