Research On Anatomical Features Of Accessory Renal Artery On CT Scan In Chinese People With Accurate Medical Demand

Objective:To satisfy clinical surgery needs,using CT image data of the accessory renal artery displayed by helical CT,combining three-dimensional reconstruction techniques,to further analyze the common anatomic characters of the accessory renal artery.To provide a reference for further realization of “fine”,"positioning diagnosis" of accessory renal artery based on CT.To provide a theoretical basis for the further realization of "smart qualitative and intelligent positioning diagnosis" of target organs after 3D reconstruction of images.Methods:Select 211 adults from Oct.2015 to Mar.2018 in the First Hospital of Shanxi Medical University,with accessory renal artery but no kidney-related diseases,underwent urinary or peritoneal enhanced CT examinations.The CT imaging data were reconstructed using 3D-Mimics software to obtain three-dimensional images.Observed in the three-dimensional image of the different types of the collateral branches from accessory renal arteries,the number of the branches,the positon of the branches emit,and the relationship with ipsilateral renal artery,the relationship between the right renal artery and the inferior vena cava and the location of renal entry.Right and left accessory renal arteries(diameter),the presence of the accessory renal arteries,left and right renal arteries(diameter),the position of right accessory renal artery,and the distance from the ipsilateral renal artery(RRAD)were measured.Results:Of 211 patients,163 were male(77.25%)and 48 were female(22.75%).In total,there were 297 accessory renal arteries and 147 right branches(49.49%),of which 66 were located in the renal artery,81 in the lower part,112 in the dorsal vein of the inferior vena cava,and 35 in the ventral.The left side was 150 branches(50.51%),among which there were 71 branches above the renal artery and 79 below.There were 143 cases(67.77%)of one branch,51 cases(24.17%)of two branches,16 cases(7.58%)of three branches,and 1 case(0.47%)of four branches.There were no five or more branch type.For the opening,4 branches originated from the common iliac artery,1 from the Inferior phrenic artery,1 from the Superior mesenteric artery and the rest from the abdominal aorta.In terms of the renal shape,46 branches pass through above the renal portal to the kidney,21 kidneys pass directly below the renal hilum,and the rest enter the kidney through the renal portal.The average maximum diameter of the left accessory renal artery was(3.67±1.03)mm.The average diameter of the renal artery was(7.48±0.90)mm when there was no accessory renal artery on the left.The mean maximum diameter was(6.31±1.09)mm with a renal artery in the left side of the accessory renal artery,the average maximum diameter of the right renal artery was(3.64±0.97)mm when emitted from the right renal artery,and the average diameter of the renal artery was(7.39±0.97)when the right renal artery was absent.The average maximum diameter of the renal arteries in the presence of the accessory renal arteries on the right side was(6.36±1.01)mm.The mean maximum diameter of the male accessory renal artery was(3.68±1.03)mm;the average maximum diameter of the female accessory renal artery was(3.60±0.89)mm.The average maximum diameter of the accessory renal arteries entering the kidney through the renal portal was(3.85±0.96)mm,and the mean maximum diameter of the accessory renal artery which not go through renal hilum into the kidney was(3.01±1.87)mm.The average diameter of the accessory renal artery directly enter above the renal portal is(2.84±0.75)mm,and the average maximum diameter of the accessory renal artery straight below the renal hilum is(3.37±1.02)mm.Of 81 branches of the accessory renal arteries,the average RRAD of the 35 accessory renal arteries ventral to the inferior vena cava was(59.13±29.21)mm.The average RRAD of 46 the accessory renal artery on the dorsal side of the vena cava was(6.27±4.48)mm.Conclusion:In this study,according to the patient's imaging data,detailed and accurate measurements of the anatomical characteristics of the adult's accessory renal arteries,such as the origin,number of branches,shape and position of the kidney.Statistically,there is a significant difference in diameter of the renal arteries with the presence of the accessory renal arteries.When the diameter of the renal artery is less than 6.615 mm,it indicates the presence of the accessory renal artery.When the right accessory renal artery was shaped on the different sides of the inferior vena cava,the difference in mean RRAD is significant statistically.When the RRAD was greater than 17.635 mm,it was more likely that the accessory renal artery was shaped in the ventral side of the inferior vena cava.Improve the detection rate of the patient's accessory renal artery,accurate understanding of the types of variations,to provide valuable preoperative guidance for various urological surgeries.
