Objective:To explore the significance of changes in the level of platelet indicators,detect the levels of platelet count(PLT),platelet volume(PCT),mean platelet volume(MPV)and platelet distribution width(PDW)in the serum of hepatitis B patients with cirrhosis and hepatocellular carcinoma,and combine the level of alpha-fetoprotein(AFP),to comprehensive analysis the clinical significance of the diagnosis of hepatitis B liver cirrhosis with liver cancer,in order to improve its diagnosis.Methods:A total of 43 patients with hepatitis B liver cirrhosis complicated with hepatocellular carcinoma(hereinafter referred to as liver cancer group)were selected,and 36 patients with hepatitis B cirrhosis and 36 patients of control group were collected;the levels of PLT,PCT,MPV and PDW in each group were obtained by a hematology analyzer;AFP Level detected by radioimmuno assay.Results:1.AFP in liver cancer group was 351.93±95.53 ng/mL,which was significantly higher than AFP in cirrhosis group(214.81±74.74ng/m L)and control group(2.52±1.14ng/m L).The difference was meaningful(p<0.05).AFP in the liver cirrhosis group was significantly higher than that in the control group(p<0.05).PLT in the liver cancer group was 222.49±75.72 x10~9/L,which was significantly higher than that in the cirrhosis group(65.83±27.42x10~9/L)and the control group(185.09±48.99x10~9/L);PLT was significantly lower in the liver cirrhosis group than that in the control group(p<0.05).Compared with the control group(0.19±0.04),PCT in the liver cancer group(0.23±0.84)was higher,than that in the control group,but the difference between the liver cancer group and the cirrhosis group(0.19±0.08)were not statistically significant(p>0.05),and the liver cirrhosis group between the control group(p>0.05).The MPV was 14.59±2.83 fL in the liver cancer group,higher than that in the cirrhosis group(11.23±1.23 fL)and in the control group(10.25±1.46 fL).Comparison between the three groups,the difference were significant differences(p<0.05).The PDW in the was 15.85±2.59 fL and in the was15.32±2.74 fL,which were both lower than that in the control group(19.97±5.42 fL). There was no difference between the liver cancer group and cirrhosis group(p>0.05).2.The MPV of AFP-negative patients was lower than that of AFP-positive patients (p<0.05);PLT,PCT,PDW and other parameters were not significantly different between the two groups(p>0.05).3.In the diagnosis of liver cancer,the sensitivity of AFP is 97.2%and the specificity of AFP is 73.6%;the sensitivity of PLT in the diagnosis of liver cancer is 97.2%and the specificity is 55.6%;the sensitivity of PCT in the diagnosis of liver cancer is 55.6%and the specificity is 75%;The sensitivity of MPV in the diagnosis of liver cancer was 75%and the specificity was 89%;the sensitivity of PDW in the diagnosis of liver cancer was97.2%and the specificity was 9.7%.4.The sensitivity of parallel combined detection of AFP and platelet four is the highest,reaching 98.30%.The specificity of the combined detection of the four parameters of AFP and platelet was up to 74.55%.Conclusion:With the gradual progression of hepatitis B liver cirrhosis to hepatocellular carcinoma,liver cancer cells can produce certain biologically active substances and body compensatory responses.PLT,PCT,and MPV levels are lower in the liver cirrhosis group than that in the hepatitis B patients with cirrhosis and hepatocellular carcinoma,but there was no significant differences of the PDW level between the cirrhosis and liver cancer groups.In the liver cancer group and the liver cirrhosis group,observing the difference of the four parameters of platelet,it can be seen that the combination of platelet four items based on the level of alpha-fetoprotein can improve the diagnosis of hepatitis B patients with cirrhosis and hepatocellular carcinoma.However,studies on platelet-related parameters in patients with AFP-negative liver cancer still need further study.
