Objective:To investigate the role of streptococcal infection in the pathogenesis of Henoch-Schonlein Purpura and Henoch-Schonlein Purpura Nephritis in children,and to analyze the correlation between Antistreptolysin O and white blood cell count,C-reactive protein,plasmaalbumin,globulin,serumcreatinine,ImmunoglobulinA,Immunoglobulin G,Immunoglobulin M,Immunoglobulin E,complement C3,complement C4,urinary protein,and urinary occult blood,and to evaluate the relationship between streptococcal infection and clinical features.Methods:A retrospective study was conducted on 154 hospitalized children diagnosed with Henoch-Schonlein purpura from April 2014 to December 2017 in the Department of Pediatrics of the First Hospital of Shanxi Medical University.The antistreptolysin O(ASO)titer was determined during hospitalization.According to the ASO titer,the children were divided into two groups.The age,season of onset,duration of skin rash,number of rash recurrences,duration of joint pain,and duration of abdominal pain were recorded.Children'swhitebloodcellcounts,C-reactiveprotein,plasma albumin,globulin,serum creatinine,IgA,IgG,C3,C4,urine protein,urine occult blood equivalent were also measured.The statistical software SPSS22.0 was used to analyze the data,analyze the differences between the two groups of data,and carry out correlation analysis between ASO and other data was performed.,and combine clinical features to analyze the significance of these data.Results:1.Among 154 children with HSP,52 cases had streptococcal infection,accounting for 33.8%;2.The age,serum globulin,creatinine,IgA values in the ASO positive group were higher than those in the ASO negative group(P<0.05),the difference was statistically significant;The mean rank of urine occult blood in ASO positive group was 96.99,the mean rank of urine occult blood in ASO negative group was 54.5,P<0.05,the difference between the two groups was statistically significant;3.There were 17 cases(32.7%)were diagnosed with HSPN in 52 cases of HSP children with Streptococcus infection,and there were 24 cases(23.5%)were diagnosed with HSPN in 102 cases of HSP children without Streptococcus infection,the two groups were compared,?~2=1.480,P=0.224,the difference was not statistically significant;4.There was a positive correlation between ASO titer and age,globulin,serum creatinine,IgA,IgG and duration of joint pain in children with HSP.Conclusion:1.Streptococcus infection is associated with the onset of HSP in children,but the occurrence of HSPN is not obvious;2.Streptococcus infection may cause immune dysfunction in children with HSP,and cause some damage of renal function.3.Children with HSP with streptococcal infection are more likely to have urinary occult blood abnormalities.
