Objective:In view of the large fluctuations in blood glucose during the perioperative period in diabetic patients,this study was based on low-dose glucose infusions to monitor blood glucose changes in patients with diabetes and to evaluate the effects of propofol and sevoflurane on diabetic patients under general anesthesia.The effects of glucose tolerance provide some references for blood glucose management in clinical anesthesia.Methods:40 cases of diabetic patients who underwent lumbar posterior laminectomy and fusion cage implanted pedicle screw fixation(PLIF)elective surgery were randomly divided into two groups: group P,propofol group,Phenol and remifentanil continued to pump to maintain anesthesia depth.In group S,the sevoflurane group,sevoflurane and remifentanil were continuously inhaled throughout the procedure to maintain the depth of anesthesia.Sodium lactate Ringer's injection was infused intraoperatively,and glucose was infused at a rate of(0.05 g / kg / h)after induction of anesthesia.The patient's intraoperative heart rate,blood pressure,pulse oxygen saturation,BIS,end-tidal carbon dioxide and other monitoring indicators were recorded;the patient was in the preparation room(FBS0),before the induction of anesthesia(FBS1),when the skin(FBS2)The blood glucose levels of 30 minutes,60 minutes,and 90 minutes(FBS3? FBS4? FBS5)were recorded.Results:There was no significant difference in age,BMI and duration of diabetes between the two groups(P> 0.05).Before the preparation,before induction of anesthesia,There was no significant difference in blood glucose between the two groups(P> 0.05).However,the blood glucose in propofol group was significantly lower than that in sevoflurane group at 60 and 90 minutes after the star of surgery,and the difference was statistically significant(P<0.05);In the propofol group,there was no significant difference between the time point and the preoprative blood glucose level(P>0.05).There was no significant difference between the sevoflurance group before anesthesia induction ande skin incisin and the preoprative blood glucose level(P>0.05).But30,60,90 minutes after the start of surgery compared with preoprative blood glucose level,the difference was statistically significant(P<0.05).Conclusin:Propofol is able to avoid intraoperative blood glucose fluctuations by injecting0.05g/kg/h doses of glucose to ensure the energy supply during surgery.Compared with propofol,sevoflurane has a lower glucose tolerance and a higher perioperative blood glucose level.Therefore,propofol can be recommended as a suitable drug for general anesthesia in diabetic patients.
