Objective:CTA in the patients which treatment effect and prognosis with spontaneous subarachnoid hemorrhage is an important means of clinical examination,and also brain parenchyma without history of hypertension hemorrhage need to exclude vascular anomalies are also widely applied,but the early hypertensive intracerebral hemorrhage were not vascular abnormalities in screening,that is generally carried out the application value of CTA have not been Effective embodiment,this research investigated CTA in high blood pressure brain have the application value of bleeding patients,to provide a meaningful reference for the clinical.Method:Total 127 cases of hypertensive cerebral hemorrhage patients as experimental group examined by CTA were collected from Department of neurosurgery in Weifang Brain Hospital from January 2015 to June 2017.According to the inspection group standards,a uniform and quantifiable statistical indicator including the patient's gender,age,bleeding side,amount of bleeding,and awareness status at admission(Glasgow Outcome Scale Score,GCS score)was established as statistical indicator of admission.Another 120 patients with hypertensive intracerebral hemorrhage as the contrast group were collected according to the same statistical indicators.We performed CTA examination on the 127 experimental group patients in the early stage of cerebral hemorrhage(within 3 days),analysis of test results,a vascular anomaly is positive,no vascular abnormalities were observed in CTA negative,check out the lesion morphology,size,location and other indicators,at the same time with DSA and surgical examination were comparison;The 13 patients with positive results were given active treatment from experimental group.Another 114 patients with negative result results were given the same treatment as the control group.The patients with surgical criteria were given craniocervical hematoma removal or hematoma drilling.The other patients were given medical treatement.We evaluated the treatment effect according to rebleeding and post-treatment score after 6 months.compared the prognosis of experimental groups can benefit from CTA examination.Analysis the data by SPSS20.0,measurement data with themean standard deviation of that between the two groups were compared by t test,count data were compared by chi square test;The difference was statistically significant if P < 0.05.Result:127 cases of hypertensive cerebral hemorrhage patients were check by CTA,the findings of CTA with positive were detected in 13 cases,4 cases of aneurysm patents(5aneurysms,including 4 unruptured aneurysms,1 ruptured aneurysms,1 cases of multiple aneurysms artery bilateral common carotid artery aneurysm unruptured),arteriovenous malformation in 3 cases,6 cases of moyamoya disease,all diagnosis by DSA and surgery,the detection rate was 10.24%(13/127).Two groups of patients with rebleeding:experimental group in January 2 cases of rebleeding,six months and 3 cases of hemorrhage,the control group in January 8 cases of hemorrhage,10 cases of hemorrhage within six months.CTA examination group was significantly lower than the control group,the difference was obvious difference(P< 0.05).The experimental group patients(4.09±0.77)had a higher score than the control group(3.68±0.97)according to the GOS score after 6months of treatment,and the difference was statistically significant(P< 0.05).Conclusion:1 We can obtain from the vascular images of high quality By CTA three-dimensional reconstruction,CTA can be used as the preferred hypertensive cerebral hemorrhage,that is safe,noninvasive and reliable method of inspection.CTA have the important clinical value in screening in hypertensive cerebral hemorrhage of checkitems,that is worthy of popularization and application.2 CTA has more important clinical guidance in the treatment of hypertensive intracerebral hemorrhage.
