Objective: To investigate the expression of heparanase(Hpa)and D2-40 in children(age? 18 years)with differentiated thyroid carcinoma(DTC)and its clinical significance,and to analyse the association with this cancer in children so as to provide some reference for the diagnosis,treatment and prognosis of DTC in children.Methods:The SP immunohistochemistry was used to detect the expression of Hpa and D2-40 in children and adults with differentiated thyroid carcinoma.The expression of Hpa and D2-40 in children with differentiated thyroid carcinoma and its clinical significance were analysed.PBS was used to replace the primary antibody as a negative control,and a known positively expressed tissue specimen was used as a positive control.All sections were observed by two pathologists under an optical microscope with the double-blind method.Each section was continuously observed under 5 high power fields of view and 100 cells were recorded under each field.A semi-quantitative analysis was performed to give scores to cytoplasmic staining intensity and percentages of positive cells.The staining intensity was graded based on the staining colours of most cells,0point for no staining,1 point for pale yellow,2 points for dark yellow and 3 points for brownish yellow.Percentages of positive cells: negative(-)0 point for 0~5%,positive 1point for 6~25%,weak positive(+)2 points for 26~50%,moderate positive(++)3 points for 51~75%,and strong positive(+++)4 points for 75% and above.If the staining results are inconsistent,the average value of cells in the 5 stained sections will be taken.SPSS20.0 statistical software was used to analyse the experimental data.A relative index descriptive analysis was performed for the count data.The x2 test was used to analyse differences between groups.P<0.05 was considered statistically significant.Result:1.The expression of Hpa and D2-40 in children and adults with DTC: Hpa and D2-40 showed strong positive expression in children with DTC,and the expression rates of Hpa and D2-40 were 96.67% and 93.33%,respectively.Hpa and D2-40 showed strong positive expression in adults with DTC,and the expression rates of Hpa and D2-40 were83.33% and 86.67%,respectively.There was no significant difference between the two groups(P>0.05).2.The relationship between the expression of Hpa and D2-40 in children with DTC and the presence or absence of lymphatic metastasis: The positive rate of Hpa in the group with lymph node metastasis was 100% and the rate in the group without lymph node metastasis was 80%;the difference between the two groups was statistically significant(P<0.05).The positive rate of D2-40 in the group with lymph node metastasis was 92% and the rate in the group without lymph node metastasis was 60%;the difference between the two groups was statistically significant(P<0.05).3.The relationship between the expression of Hpa and D2-40 in children with DTC with sex,histological types,capsular invasion or other factors:The positive rate of Hpa in papillary thyroid carcinoma was 100% and the rate in follicular thyroid carcinoma was80%;the difference between the two groups was statistically significant(P<0.05).4.Therefore,Hpa and D2-40 were highly expressed in both children and adults with DTC,and the expression of the two proteins in children with thyroid cancer was closely related to the degree of lymph node metastasis(P<0.05).Among them,the expression of Hpa was also related to the histological type of tumours,but it was unrelated to age,sex and capsular invasion.Conclusion:Hpa and D2-40 were highly expressed in children with DTC and were closely related to lymph node metastasis.Therefore,the characteristics may be used as biological indicators for children with DTC,which provides a certain value for the clinical evaluation and postoperative management in such children.
