Study On The Correlation Between Nonalcoholic Fatty Liver Disease And Senile Degenerative Valvular Heart Disease

Objective: To explore the relationship between nonalcoholic fatty liver disease and senile degenerative heart valvular disease,and to develop new ideas for prevention and treatment.Methods: A retrospective analysis was made of 1098 elderly patients aged more than 60 years old and aged over 60 years old in the health care department of the Affiliated Hospital of Qiingdao University from September 2015 to March 2018.At the same time,840 patients were perfected by echocardiography,abdominal ultrasound and blood test.According to the results of abdominal ultrasound,they were divided into group NAFLD(including 457 cases with SDVHD,133 cases without SDVHD,630 cases in total)and non NAFLD group(210 cases).At the same time,the patients in group NAFLD were divided into mild group(210 cases),moderate group(210 cases)and severe group(210 cases),according to the difference in the liver accumulation of fat under the abdominal ultrasound.In addition,according to the difference of heart valve involvement and number of echocardiography,the heart valve was divided into SDVHD group(210 cases involving one valve HCV-1,287 cases involving two and more valve HCV-2)and non SDVHD group(heart valve unaffected and HCV-0 133 cases).To analysis of the difference in constituent ratio of elderly people with NAFLD due to different sex and SDVHD;Is there any difference in the constituent ratio of SDHVD patients with different severity of NAFLD? Is there significant difference between the NAFLD group and the control group in the severity of SDVHD? The prevalence and severity of NAFLD change with age in the elderly;By comparing the clinical and biochemical indexes of the NAFLD group and the control group with different severity,the traditional risk factors of NAFLD and SDHVD(Age),BMI,diastolic pressure(diastolic blood pressure,DBP),the ratio of neutrophils(Neutrophil ratio),C reactive protein,and Gu An were further analyzed.The correlation between gamma-glutamyltranspeptidase(GGT),alanine transaminase(ALT),triglyceride(TG),uric acid(UA),hypertension(hypertensive disease,HD),diabetes(diabetes mellitus,DM)hyperlipidemia(hyperlipidemia),etc.Discuss the relationship between NAFLD and SDVHD.Results: 1.Compared with the control group,the constituent ratio of SDVHD in elderly male patients in group NAFLD was significantly higher than that in control group(P?0.05);There was no significant difference in the constituent ratio of SDVHD between the NAFLD group and the control group(P?0.05).The constituent ratio of SDVHD in group NAFLD was significantly higher than that in control group(P?0.05);the constituent ratio of SDVHD was significantly higher than that of control group.The severity of SDVHD in group NAFLD was significantly different from that in control group,P?0.05.With the increase of age,the number of NAFLD patients showed a downward trend,which was negatively correlated with age.The difference was statistically significant(P?0.05).2.There was no significant difference in sex,systolic pressure,white cell count,gluten transaminase,alkaline phosphatase,leucine transpeptidase,low density lipoprotein and total cholesterol(all P?0.05)compared with the control group in the NAFLD group.Compared with the control group,the age,BMI,uric acid level and neutrophil ratio,CRP,alanine transaminase and glutamyl transpeptidase were statistically significant differences between NAFLD groups(P?0.05).Compared with the NAFLD group of different severity,the composition of hypertension,diabetes and hyperlipidemia in the group of light and medium recombined NAFLD was higher than that of the group(P?0.05).The SDVHD combination and NAFLD severity were significantly different from those in the control group,P?0.05.3.Multiple factor Logistic regression analysis showed that age,sex(male),body mass index,diastolic pressure,neutrophil ratio,uric acid,NAFLD,hyperlipidemia,and hypertension were independent risk factors of SDVHD,and the relative risk of NAFLD was higher(p=0.000,OR= 9.720).Conclusion: 1.Elderly men with NAFLD are more likely to develop SDVHD than elderly women.The prevalence of NAFLD in elderly patients is negatively correlated with age.2.The elderly NAFLD is a systemic disease.The severity of NAFLD is closely related to the risk factors of SDVHD,such as hypertension,diabetes,hyperuricemia,age,BMI,GGT,and mutual influence,together to promote the progress of SDVHD and NAFLD in the elderly patients.3.NAFLD is an independent risk factor for SDVHD.
