Objective:Cerebrovascular disease is one of the most common causes of death in the world. Its morbidity, mortality, morbidity, and relapse rate are all high. The three leading causes of death in humans include heart disease, malignancy, and cerebrovascular disease.According to the related epidemiological data in recent years, cerebrovascular disease ranks the first and the second in China's population death causes.Compared with western developed countries, the current incidence and mortality of cerebrovascular diseases in China are significantly higher than those of cardiovascular diseases.Ischemic stroke accounts for 70% of all cerebrovascular diseases and is the most common type of cerebrovascular disease.Traditional studies suggest that the most important cause of ischemic stroke is caused by intracranial atherosclerotic plaque, but with the economic and social development, the incidence of ischemic stroke tends to be younger, and that of elderly patients is intracranial. The most common cause of arterial stenosis is intracranial atherosclerosis, and the cause of intracranial arterial stenosis in young patients is not only intracranial atherosclerosis, but there are also multiple causes, such as dissection, moyamoya disease, and fibromuscular dystrophy.Symptoms,vasculitis,etc.However,the main causes of intracranial arterial stenosis in young patients are still rare and reliable reports at home and abroad. The main reason is that the pathological examination of intracranial blood vessels is difficult to obtain, and pathological analysis of intracranial vessels cannot be performed. However, with the development of imaging, the current understanding of cerebrovascular diseases has developed to high resolution magnetic resonance(HRMRI).It is a new imaging technology that can directly observe the pathology of the blood vessel wall to guide clinical prevention and treatment. In this study,HRMRI,an emerging imaging tool, attempts to explore the causes of intracranial arterial stenosis in young patients. Methods :Based on inclusion and exclusion criteria, from August 2016 to October 2017,95 cases of symptomatic ischemic cerebrovascular disease admitted to the Department of Neurology,Yuhuangding Hospital,Yantai City were investigated. After admission, patients undergo blood biochemistry, blood routine and urine routine examinations, and collect baseline data such as age, gender, and medical history.Magnetic resonance imaging(MRI)examinations are then performed to determine the diagnosis of transient ischemic attack(TIA)and cerebral infarction.Magnetic resonance angiography(MRA)examines vascular conditions and uses responsible blood vessels as target blood vessels. If there is a multi-vessel stenosis, the symptomatic blood vessel will become the target blood vessel.According to the target vessel selected by MRA,HRMRI examination was performed to analyze the vascular stenosis of the target vessel. The clinical data and vascular wall characteristics of patients over the age of 35 and over 35 were compared. The chi-square test was conducted using SPSS 22.0 statistical software. The test results were statistically significant at P < 0.05.Finally,use binary logic analysis to prove concentricity. Results: 1.Of the 95 specimens prepared in this study,65 patients(68.4%)had eccentric stenosis and 30 patients(31.6%)had concentric stenosis.Patients with eccentric stenosis were older(35.3 ± 10.6 vs 30.1 ± 6.4 years,p <0.05)and were more likely to develop risk factors for atherosclerosis: hypertension(72.3% vs.56.7%,P = 0.001).2.In its subgroup analysis,23 patients were younger than 35 years old and 72 patients were > 35 years old.Compared with patients younger than 35 years, atherosclerotic hypertension(P = 0.001),smoking(P = 0.004),and eccentric stenosis(P <0.001)were not associated with the type of stenosis in patients aged ? 35 years.P = 0.38).3.In the logistic regression analysis, there was a negative correlation between age-independent effects and negative stenosis. The older the age of onset, the less likely it is that concentric stenosis will occur;the positive changes in smoking and concentric stenosis and the possibility of concentric narrowing of smoking.3.69 times the number of smokers. Conclusions:The most common cause of intracranial stenosis in young Chinese is still atherosclerosis.Gender has nothing to do with the type of stenosis.
