Objective To study the epidemiology of bacterial infection in hospitalized patients with hematological diseases in Department of Hematology,Affiliated Hospital of Qiingdao University in recent 10 years.Then investigate the clinical characteristics of bacterial infection in hospitalized hematological patients in Department of Hematology,and analyze the distribution and changes of the pathogens isolated and antimicrobial susceptibility.METHODS In our study,a retrospective analysis was used to collect data from all patients who were hospitalized in the Department of Hematology at the Affiliated Hospital of Qiingdao University from January 2006 to December 2015 and had a bacterial infection in the patients.The samples were derived from blood,middle urine,pharynx swab,sputum and so on.The distribution of bacteria,drug sensitivity,risk factors,clinical characteristics and the change trend of the infection were statistically analyzed.RESULTS There were 2338 times of infection,and the infection rate was 18.4%.Among them,patients with malignant hematopathy accounted for 88.5% of all infections.A total of 852 isolates were isolated,and the positive rate of pathogen isolation was 47.1%.Gram negative bacteria(469 strains)accounted for the largest number,accounting for 55% of all pathogenic bacteria,and 166 Gram-positive bacteria(19.5%).In the past 10 years,the infection rate of Gram-negative bacteria has been rising or falling.The overall incidence of Gram-negative bacteria has increased.The rate of Gram-positive bacteria has been steadily rising.The top 10 bacteria were Escherichia coli(15.3%),Klebsiella pneumoniae(9%),Pseudomonas aeruginosa 70(8.2%),Acinetobacter baumannii(6%),45 strains of Staphylococcus(5.3%),41 strain of Pseudomonas maltophilia,Staphylococcus aureus,Staphylococcus epidermidis,Staphylococcus epidermidis,and gutters.30 strains(3.5%)and 17 Enterococcus faecium strains(2%).In addition to the natural resistance of Stenotrophomonas maltophilia to carbapenems,the other major Gram-negative bacteria were more sensitive to meropenem and imipenem.4(5.8%)Pseudomonas aeruginosa were found to be multidrug-resistant bacteria.Except for 1 strains of Staphylococcus epidermidis resistant to teicoplanin,the other gram positive bacteria were sensitive to vancomycin,teicoplanin and linezolid.CONCLUSIONS Hematological diseases,especially those with malignant hematologic diseases,have a high risk of infection and a high infection rate.However,with the improvement of medical level and awareness of infection prevention,the infection rate of the patients with hematological diseases is decreasing.Gram negative bacteria are the most common pathogenic bacteria causing bloodstream infections.The problem of bacterial resistance is increasing,and multidrug-resistant bacteria appear.
