Objective Psoriasis is a common chronic inflammatory dermatosis,the prevalence of which is about 1 %,but has been increasing in recent years.Because the disease incidence rate increases year by year,and the course of the disease is longer,difficult to cure,easy to repeated attacks,seriously affect the patient's physical and mental health.The causes of psoriasis are complex,and the most important pathogenesis is the abnormality of immune cells and molecules.Although there are reports about the single immune pathogenesis of psoriasis at home and abroad,the results are different.This may be due to different cases,different psoriasis types and inconsistent detection methods.In this study,we selected patients with psoriasis vulgaris as subjects to observe the levels of T lymphocyte subsets(CD3 + T cells,CD4 + Th cells,CD8 + TC cells,T helper cell 17(Th17),regulatory T cell(Treg))and the levels of IL-17?IL-4?IL-10?IFN-?and IL-6.To explore the mechanism of immune abnormality in psoriasis from the cellular and molecular levels,so as to provide new methods and ideas for clarifying the pathogenesis of psoriasis and developing clinical target drugs.Methods 60 cases of psoriasis(experimental group)and 30 cases(control group)of medical examination in our dermatology center from October 2015 to June 2017 were selected.The proportions of CD3 + T cells,CD4 + Th cells,CD8 + TC cells,Th17 and Treg cells in the peripheral blood of the subjects were measured by flow cytometry.The concentrations of Interleukin-17(IL-17)and Interleukin-10(IL-10)were detected by enzyme-linked immunosorbent assay(ELISA).and plasma levels of IFN-?(Th1)and Interleukin-4(Th2)were detected by Cytometric Beads Array(CBA).The level of serum interleukin-6(Interleukin-6)was determined by electroluminescence immunoassay.Results 1.The percentage of CD4 + Th cells in peripheral blood of psoriasis patients was significantly lower than that in healthy control group,and the percentage of CD8 + TC cells was significantly higher,with significant difference(P< 0.05).However,the ratio of CD3 + T cells and CD4+ / CD8+ was lower than that of healthy control group,but there was no significant difference(P >0.05).2.The percentage of Th17 cells,the level of IL-17 in serum were higher in psoriasis patients than in healthy controls(P< 0.05),the ratio of Th17 cells to Treg cells was significantly higher in psoriasis patients(P< 0.01).However,the proportion of Treg cells and the level of IL-10 in serum decreased significantly compared with the healthy control group(P<0.05).3.The level of IFN-gamma in serum of psoriasis patients was significantly higher than that in healthy controls(P< 0.01),and the level of IL-4 was significantly lower than that of healthy control group,and there was significant difference(P<0.05).4.The level of IL-6 in psoriasis group increased significantly,and the difference was statistically significant(P< 0.01).Conclusions 1.This study confirms that in patients with psoriasis Th2 drifts toward Th1,and the number and function of Th1(IFN-gamma)in patients were increased.Th2(IL-4)decreased in number and function,did not antagonize IFN-gamma mediated cell immunity,and aggravated psoriasis development.2.The ratio of Th17 cells to Treg cells in psoriasis patients was unbalanced.The levels of Th17 cells and secreted cytokines IL-17,IL-6,and IFN-gamma were increased,while the number of Treg cells with inhibitory effects decreased,the inhibitory cytokine IL-10 content decreased,which induced the infiltration and proliferation of inflammatory cells and T lymphocytes in the lesional area,resulting in persistent attacks of psoriasis and recurrent attacks.3.The cause of psoriasis is complex.It is the result of multiple factors.In particular,the imbalance of immune cells and abnormal expression of immune molecules are the main mechanisms of psoriasis.This study provides experimental evidence for molecular immune mechanism of psoriasis,and has important theoretical significance and clinical application value.
