| Background:Anterior Cervical Discectomy and Fusion(ACDF)with plate fixation is a recognized method for the treatment of degenerative cervical disc disease.New absorbable materials can avoid disadvantages and complications caused by some metal devices.Objective:The purpose of this study was to introduce the clinical experience of the application of biological absorbable nail plate in the treatment of degenerative disease of single segment cervical intervertebral disc in anterior cervical intervertebral disc resection and fusion.The clinical efficacy of titanium alloy nail plate was compared.Methods:Two groups of patients were treated with absorbable nail plate(n = 11,10 male and 1 female,mean age 51.9 years)and titanium alloy plate(n = 28,15 male and 13 female,mean age: 52.7 years).The average follow-up was 42 months in the absorbable group and 46 months in the titanium alloy group.The cervical vertebrae cobb angle,intervertebral height and prevertebral soft tissue thickness were evaluated by radiography,and the distance between spinous process was measured by the extension and flexion of cervical vertebrae to judge the fusion rate of the operative segment.The clinical efficacy was evaluated according to visual analogue score(VAS)and the recovery of nerve function was evaluated by orthopaedic association)JOA scoring system of Japanese orthopedic society.Results:At the last follow-up,the mean ±standard deviation of cervical cobb angle in the absorbable group was 7.16 ±4.43,which was slightly larger than that before operation(p > 0.05),but it was significantly lower than that of 2 days after operation(P < 0.05).The mean ±standard deviation of cervical cobb angle in titanium alloy group was 8.7 ±8.0,which was significantly larger than that before operation(p < 0.05),and decreased slightly compared with 10.9 ±8.2 on the 2nd day after operation.The mean ±standard deviation of intervertebral height in the two groups was slightly higher than that before operation,but the intervertebral height decreased more than 0.05 compared with 2 days after operation.Two days after operation,the anterior intervertebral space of the two groups was significantly widened(P < 0.05),and the difference was statistically significant.At the last follow-up,the anterior intervertebral space was recovered in both groups.The fusion rates of the two groups were 90.9 / 10 / 11)and 96.4m / 27 / 28,respectively.There was no significant difference between the two groups(P > 0.05).The vasojoa scores of the two groups at 2 days and the last follow-up were significantly improved compared with those before operation(P < 0.05),and the difference was statistically significant.There was no difference between the two groups after operation and at the last follow-up(P > 0.05).Conclusion : The results of this long-term retrospective clinical follow-up confirmed that patients with biotype absorbable tension plates with single segment ACDF can well tolerate biotype absorbable materials.The absorbable materials can successfully achieve cervical fusion and obtain the same fusion and stability as titanium alloy nails,and no one patient happens around.Tissue swelling,infection,failure and reoperation,implant displacement and other complications.All patients in this study have achieved good results in the long-term follow-up.More cases are needed to prove the long-term effect of the biotype absorbable nail board for ACDF. |
