Objective: To analysis the difference between the 3D printed subperiosteal implant and traditional subperiosteal implant in mechanical properties;and to evaluate the healing effect of individual SI.Methods: Three dimensional model of individual subperiosteal implant and traditional subperiosteal implant was constructed,and the mechanical properties differences of the two were evaluated by three-dimensional finite element method.Digital reconstruction techniques were utilized to design an individualized subperiosteal implants with reasonable dispersion of bite force and put the guide plate in place.3D printed technique was utilized to accurately print out the implants.An experimental beagle model of bone defect was established for implant implanting operation.After the general postoperative continuous observation and the CBCT images observation,the animals were sacrificed after 3 months of the surgery.Histological analysis of the specimens was carried out to evaluate the healing effect.Results: The finite element analysis results show that the 3D printed SI has better stress dispersion performance than the traditional frame SI.All 12 SIs were fitted smoothly in accordance with the guide plate.After surgery,infrapatellar structures of the three SIs were exposed at different postoperative periods.X-rays showed diffferent degrees of bone resorption;3 SI cuffs healed well,X-rays showed no significant bone resorption,and resin crowns were repaired 2 months after the surgery.Histological examination confirmed SI as inflammatory fiber healing.Conclusion: The finite element analysis results show that the 3D printed SI has better stress dispersion performance than the traditional frame SI.The implants and bone surface can produce a certain degree of integration,especially near the retention pin.For the clinical application of 3 d printing subperiosteal implant provide a certain amount of practical experience.
