Objective:To early diagnosis sarcopenia in elderly with type 2 diabetes,especially in primary care setting,this study is aimed to explore the accuracy of the simple biomarker on diagnosing sarcopenia in those subjects.Methods:A total of 264 elderly with type 2 diabetes(age:66.11±4.43 years,male/female:98/166)from the first affiliated hospital of Chongqing medical university were recruited in this study,Skeletal muscle mass was deteced by dual energy X ray absorbmtry,and 6m usual gait speed and grip strength were also measured.Sarcopenia was diagnosed according to the criteria established by the Asian Working Group for Sarcopenia.Demographic characteristics,clinic features,nutrition indicators,inflammatory biomarkers,chemical indicators,body composition were collected and analyzed.logistic regression were used toassess the independent predictor for sarcopenia with simple physical data and biomarkers,including inflammation makers and nutritional index.Finaly,simple biomarker was deduce from those independent risk factors and analyze though the ROC curve.Results:Sarcopenia was present in 31.8%of this study population.Compared with non-sarcopenia group,diabetic patients with sarcopenia had more older,less weight and body mass index(BMI),tend to be more likely suffering from diabetic nephropathy,Diabetic peripheral neuropathy,lower bone mineral mass of femoral neck,adverse consequences of fall,but higher level of plasma calcium,hypersensitivity-C reaction protein(hs-CRP),percentage of body fat(PBF),visceral fat mass(VFA)(p<0.05).And age?BMI?hs-CRP may act as the independent risk factors of sarcopenia(p<0.05).Sarcopenia index,obtained from the diagnostic regression formula,had a high accuracy in ROC curve analysis(sensitivity 76.2%,specificity 76.1%)when the cutoff value for sarcopenia index was-2.59.Conclusions:the sarcopenia index,as a practical simple marker,may be useful in detecting sarcopenia in geriatric patients with type 2 diabetes.
