Objective:To investigate the problems associated with the exchange transfusion standard of neonatal hyperbilirubinemia and the causes of the decrease in exchange rate.Methods:Newborns admitted to the Department of Neonatology,Children’s Hospital of Chongqing Medical University from January 2011 to December 2012 and January 2016 to December 2016,and diagnosed with neonatal hyperbilirubinemia and met the standard of exchange were divided into two groups depending on whether they were treated with exchange transfusion.All the confounding factors between the two groups were balanced.Comparing the relative indexes of acute bilirubin encephalopathy,to explore whether the standard of exchange transfusion is appropriate and the reasons for the decrease of the rate of exchange.Results:The exchange rate showed an uptrend from 2001 to 2012,and decreased year by year after 2012,and the difference was statistically significant.After 1:1 matching,190 pairs were matched successfully in2011-2012 and 65 pairs were matched successfully in 2016-2017,and theconfounding factors between the two groups were all balanced.In the2011 to 2012 years,there was a significant difference in the severity of ABE between the two groups(P < 0.05),but there was no difference in the score of NBNA、BAEP、magnetic resonance imaging and hearing screening between the two groups(P > 0.05).In the 2016 to 2017 years,Neither the severity of ABE nor the score of NBNA、BAEP,magnetic resonance imaging and hearing screening have significant differences between the two groups(P > 0.05)Conclusion:The standard of exchange should be adjusted in our country,and the exchange rate was significantly reduced with the development of perinatal medicine.
