| Objective:(1)To identify symptom clusters in individuals with chronic heart failure.(2)To investigate the effect of exercise experimental on symptoms cluster and left ventricular systolic function in old patients with chronic heart failure and provide guidance for clinical practice.Methods:(1)From March 1,2016 to October 30,A sample of 201 patients who come from our hospital with chronic heart failure were investigated by the Chinese version of the Memorial Symptom Assessment Scale-Heart failure(MSAS-HF).Symptoms with the incidence above 30% were analyzed by factor analysis,also tested the severity,distress.(2)From Feb1,2017 to May 30,2017,88 elderly patients with chronic heart failure in our hospital were randomly divided into experimental group(44 cases,Using control intervention + exercise experimental intervention),control group(44 cases,conventional intervention).The study compared the,severity of fatigue emotion-related symptom cluster(out of spirits,loss of weight,Easily trapped,concentrate difficulty,anxious,distressed,feelnervous)and left ventricular systolic function in experimental group and control group.Results :(1)Five symptom clusters were identified: dyspnea-related,physical-related,fatigue-related,general and emotion-related.Severity and distress of symptoms scored 2~3.After the intervention,the severity of symptom cluster including out of spirits,loss of weight,Easily trapped,concentrate difficulty,anxious,distressed,feel nervous were lower in the experimental group,The SV and EF values of the two groups were higher than those before exercise(P<0.05),The SV and EF value of the experimental group were higher than that of the control group(P<0.05).Conclusions :(1)Symptoms cluster and symptoms in elderly patients with heart failure constitute a relatively special group,clinical medical staff on the basis of further evaluation studies,for the clinical signs and symptoms in elderly patients with heart failure group management is of great significance.Exercise experimental intervention in elderly patients with chronic heart failure can significantly reduce the severity of fatigue emotion-related Symptom cluster and and improve the patients' left ventricular systolic function. |
