BackgroundAt present,the patients with thoracolumbar tuberculosis combined with primary osteoporosis are increasing.Because of their unique disease characteristics,the treatment of such patients is a difficult point.There are few reports about simple posterior surgery for such patients.ObjectiveTo investigate the clinical efficacy of simple posterior debridement,bone graft fusion and pedicle screw fixation in the treatment of patients with primary lumbar osteoporosis.MethodsA retrospective analysis of our hospital department of orthopedics from January 2014 to October 2016,using a simple posterior surgery for thoracolumbar tuberculosis patients,a total of 75 patients met the relevant inclusion criteria.75 cases of thoracolumbar spinal tuberculosis were divided into two groups:the study group,35 patients with primary osteoporosis and 35 patients with thoracolumbar tuberculosis.The study group was treated with anti-osteoporosis treatment combined with standard anti tuberculosis treatment,and the control group was given a regular anti tuberculosis treatment.The data of two groups were analyzed,including the improvement of waist and leg pain(VAS score),the cure of thoracolumbar tuberculosis and the failure of internal fixation.ResultsThe operation of the two groups was smooth.No major complications such as nerve root injury were found.The two groups were followed up steadily.The study group consisted of 15 males and 20 females,with an average age of(62.6±12.7)years.The mean follow-up time was(20.6 + 4.8)months,and the average bone graft fusion time was(12.2 + 0.9)months.The control group consisted of 17 males and 23 females,with an average age of(34.9 + 7.4)years.The mean follow-up time was(21.9 + 5.4)months,and the average bone graft fusion time was(9.7 +1)months.Two groups of patients with postoperative lumbago and leg pain(VAS score)were better than before the operation(P<0.05).Two groups of patients with bone fusion after last follow-up,no internal fixation failure.There were different degrees of improvement in the last follow-up of patients with impaired nerve function.ConclusionThe timely and effective anti-osteoporosis drug and early combination,full,regularity and amount of anti-tuberculosis treatment as the premise,and the use of posterior debridement,bone graft fusion and clinical effects of pedicle screw fixation combined with primary osteoporosis patients with thoracolumbar spinal tuberculosis.
