The Efficacy Of Radiofrequency Catheter Ablation Of Atrial Fibrillation And The Predictors Of Recurrence

Objective:To elucidate the clinical efficacy of radiofrequency catheter ablation(RFCA)in atrial fibrillation and the predictors of recurrence.Methods:A total of 110 AF patients received RFCA were collected from January 2015 to May 2016 in the first affiliated hospital of Chongqing Medical University,including 67 patients with paroxysmal AF.The ablation techniques including bilateral pulmonary vein isolation(PVI),additional linear lesions,and complex fractionated atrial electrograms(CFAE).After ablation,patients were followed-up by ECG or Holter,and UCG.Patients were divided into two groups based on whether the arrhythmia recurrence.The changes of the RAD,LAD,LVEDD,LVEF were compared between the two groups.Logistic analysis was used to analyze the risk factors for the recurrence of atrial fibrillation after ablation.Results:The total recurrence rate was 34.55%in the first year after ablation.The paroxysmal AF group(22.39%)was significantly lower than the persistent AF(53.49%)(P=0.001).The total LAD(P=0.001)and RAD(P=0.018)decreased significantly after ablation.By multivariate Logistic analysis showed that persistent atrial fibrillation(HR 2.833,95%CI 1.119-7.171,P=0.028),heart failure(HR 3.218,95%CI 1.254-8.257,P=0.015),double atrial expansion(HR 5.151,95%CI 1.413-18.781,P=0.013)were independent predictors for recurrence after RFCA of atrial fibrillation.Conclusion:RFCA is effective in patients with paroxysmal AF.Double atrial expansion,heart failure and persistent atrial fibrillation were independent predictors for arrhythmia recurrence.RFCA can significantly reduce the size of double atrial.However,it did not reduce the LVEDD or improve the LVEF.
