Clinical Observation Of Refractory Epilepsy Children Treated With Ketogenic Diet

Objective:To study the efficiency of ketogenic diet(KD)on the children with refractory epilepsy,as well as the correlation between the efficiency and the gender,age,treatment period,epilepsy syndrome,seizure types,kinds of anti-epileptic drugs and cranial imaging.Methods:Fifty pediatric patients with refractory epilepsy who have received KD therapy are recruited,the follow-up time rang from 3 months to 24 months.We use Engel grade to evaluate the efficiency at 3,6,9,12,18 and 24 months after treatment,and we analysis the correlation between the efficiency and the gender,age,treatment period,epilepsy syndrome,seizure types,kinds of anti-epileptic drugs and cranial imaging.Results:(1)The total effective rate of the KD at 3,6,9,12,18 and 24 months is respectively 80%,75%,81.8%,88.2%,75%,and 85.7%,and the total control rate is 72.5%,75%,66.7%,80%,77.8%,66.7%,respectively.(2)There is no correlation between the efficiency and the gender,age,treatment period,epilepsy syndrome,seizure types,kinds of anti-epileptic drugs and cranial imaging.(3)The effective rate of the KD treatment in IS,LGS and FIRES are between 50%-100%,however there isno correlation between the three different types of the epilepsy syndromes.Conclusion:(1)KD therapy for children with refractory epilepsy is effective.(2)The curative effect of KD in children who are effective after 3months treanment is not significantly decreased with the prolongation of treatment time,so children should insist on KD therapy.With the prolongation of treatment time,there are only a few children with epilepsy recurrence.(3)There is no correlation between the efficiency and the gender,age,epilepsy syndrome,seizure types,kinds of anti-epileptic drugs and cranial imaging,so for the intractable epilepsy,as long as there is no contraindications,we should consider the KD therapy.(4)In our study,the effective rate of the KD treatment in IS,LGS and FIRES are between50%-100%,however there is no correlation between the three different types of the epilepsy syndromes.(5)In future,we need more samples to analyse the relationship between the efficacy of KD and the types of epilepsy and seizure,the interaction between the KD and the AEDs,and the influence factors of epilepsy relapse after KD treatment.
