Objective: 1.To explore the differences between HRMRI(high-resolution magnetic resonance imaging)and CTA(computed tomography angiography),and DSA(digital subtraction angiography),when using them on the patients of acute cerebral infarction to study the abnormalities of intracranial vessels.2.To explore the influence of HRMRI on the etiological classification of patients with isolated penetrating artery infarctions.Methods: The clinical data of acute cerebral infarction patients who have taken HRMRI in the Second Affiliated Hospital of Chongqing Medical University from March 2016 to September 2017 were retrospectively analyzed.1.Detecting the intracalvarium of internal carotid arteries,M1 segment of the middle cerebral arteries and basilar arteries,and comparing the difference of detection rate(intracranial plaque or stenosis)in HRMRI and CTA group,HRMRI and DSA group.2.Based on China ischemic subclassification,the patients of isolated penetrating artery infarctions underwent the HRMRI,if there is no evidence of atherosclerotic plaque in the target arteries detected by HRMRI,they were classified into the HRMRI negative group.If there is any evidence of plaque,they were classified into HRMRI positive group.Comparing the clinical characteristics between HRMRI positive group and negative group.Results: 1.A total of 78 patients were included in the study between HRMRI and CTA,390 arteries were studied.The detection rate of class 1,class 3 and class 4 in HRMRI is significant higher than CTA group(P<0.05),and there was no significant difference in class 2 between the two groups(P>0.05).2.A total of 31 patients were included in the study between HRMRI and DSA,155 arteries were studied.The detection rate of class 1 and class 3 in HRMRI is significant higher than DSA group(P<0.05),and there was no significant difference in class 2 between the two groups(P>0.05).3.A total of 48 patients were included,who has isolated penetrating artery infarctions.There are 26 patients with plaque in the target arteries detected by HRMRI,classified into the HRMRI positive group;22 patients with no evidence of plaque detected by HRMRI were classified into the HRMRI negative group.The average age and low density lipoprotein in HRMRI positive group are significant higher than HRMRI negative group(P<0.05),and there was no significant difference in other clinical characteristics between the two groups(P>0.05).Conclusion: 1.Comparing to CTA and DSA,HRMRI can provide more information of intracranial vessels,and can detect the vessel stenosis in clinical use.2.HRMRI can help the etiological classification of patients with isolated penetrating artery infarctions.
