| Objective: Periodontitis is a chronic progressive disease caused by a variety of causes,and plaque biofilm is the initiating factor.Endotoxin is the lipopolysaccharide(Lps)in the outer membrane of gram-negative bacteria.it plays an important role in the development of periodontal disease.Experimental observation found that,twinlight laser treatment of tooth root surface with severe chronic periodontitis.The effect of dual wavelength laser on the treatment of chronic periodontitis was evaluated by observing the changes of root surface morphology and endotoxin concentration.Methods:Detection of endotoxin concentration.1.Subjects: 64 molars with no retained value due to severe chronic periodontitis.The teeth along the axis of the tooth are divided into two halves in the proximal symmetry.10 normal premolars were removed due to orthodontic causes,and the bone fragments were prepared according to the requirements.2.Experimental group: according to the characteristics of the specimen and the treatment method,the bone fragments were divided into the following groups:Group A: normal control group(n=10);One tooth of each tooth is nottreated with root surface into Group B: periodontitis group(n=64,positive control group);The teeth of periodontitis were randomly divided into:Group C: SRP(scaling and root planning)(n=16);Group D: Er: YAG Laser(Erbium laser)group(n=16);Group E: Er: YAG +Nd: YAG Laser(neodymium Laser)group(n=16);Group F: Er: YAG Laser+ semiconductor Laser group(n=16);3.experimental methods: all samples of periodontitis were randomly assigned and numbered.Remove the periodontal membrane.According to the requirements,4mm x 4mm x 1mm of the size of cementum 2 tablets was taken at 2mm under EDJ.Orthodontic teeth preparation of bone fragments for use.in group C.D.E.F,6 samples were randomly selected for sem observation.The samples of group B only removed the soft scale and soft tissue,and the endotoxin concentration was detected directly.The other one was treated with root surface according to the group,and the endotoxin concentration was detected.then detection endotoxin concentration.The changes of endotoxin concentration after different treatment were recorded for each number of periodontitis.Results:In group A and group B all sample has no statistical difference(P> 0.05),compared with periodontitis group,all the endotoxin concentration in the treatment group were significantly lower and statistically difference(P< 0.05),in the treatment group,the highest concentration of sample is group C,C and A,D,E,F between groups were statistically difference(P < 0.05),minimum concentration in group F,but with A,D,E,has no statistical difference(P>0.05),and A,D,E,F group has no statistical difference between groups(P>0.05).There was no statistical difference between the concentrations of endotoxin in the treatment group.D,E,and F(P>0.05),and there were statistically significant differences in group C(P<0.05).In the sample of group A,there is A large amount of defiled layer and small floss scattered in the sample.As well as the uniform crack in the same direction as the scraping direction.and it can be seen that the trace of cementum and partial regional dentin were exposed.There was no fusion,carbonation and fissure in group B,and there was no obvious trace of cementum and dentin,but the dentinal tubule was open in some areas.No carbonation and cracks were seen in group C,and the contamination layer existed,the exposed dentin tubules were visible and partially closed.In group D,no cracks were found in the samples,and the stain layer existed,and some areas were carbonized,and the exposed dentinal tubules were visible and most completely closed.Conclusion:1.Hand instrument and laser treatment can effectively remove the odontolith and endotoxin of chronic periodontitis.2.Compared with hand equipment,laser treatment is safe and reliable,and it can better save the root surface dental tissue.3.Dual wavelength laser compared with Er: YAG laser,there was no significant difference in the removal of endotoxin. |
