Objective:High-grade squamous intraepithelial lesion(HSIL)is a precancerous lesion with a significant risk to become invasive cancer.The study is aim to summarize the postoperative pathological characteristics of cervical high-grade intraepithelial lesions,then to provide guidance and reference for clinic.Methods:1 306 cases with cervical HSIL(mainly CINⅢ,confirmed by cervical biopsy under colposcopy),were collected from February 2012 to August 2017in the 4~thh hospital of Hebei Medical University.By analyzing the related clinical data,the patients with HSIL were evaluated for postoperative pathology and the relevant factors.2 Statistics:SPSS 22.0 statistical software was used.x~2 test was used to analyzethe relationship between postoperative pathology and the related clinical follow-up indicators(patients’age,medical reasons,TCT,HPV,examination of department of gynaecology,gynecological B ultrasonic,tumor markers,intraoperative situation).P<0.05 was considered significance.Results:1.Among 306 patients with high-grade cervical intraepithelial lesion,there were 131 cases diagnozed as cervical cancer,incidence 42.48%.The postoperative pathology after cervical conization in 176 patients was benign or precancerous lesions.2.The related factors analysis of postoperative pathology of HSIL1)Age:The postoperative pathology of HSIL was related with the age.With the increase of the age,the incidence of postoperative cancer increased.The difference was significant(P<0.05).2)HPV:The postoperative pathology of HSIL was related with the HPV infection.Among HPV positive patients,the possibility to become cancer increased,significant difference(P<0.05).3.The related factors analysis of postoperative pathology of HSIL with age>50The increase of SCC,TCT result as ASC,and HPV infection had the close correlation with postoperative pathology of HSIL.The difference was significant(P<0.05).Conclusion:1.The probability to become cancer is high after cervical conization in older HSIL patients diagnosed by cervical biopsy under colpcopy.2.HPV infection is a risk factor to influence the postoperative pathology as cancer of HSIL.3.The increase of SCC,HPV infection and TCT result as ASC are the risk factors to influence postoperative pathology of HSIL patients over age 50.4.It is necessary to do deeper examination for HSIL patients with risk factors.It is possible to get exact diagnosis to do hysteroscopy and abdominal MRI examination,then to prevent the accidental injury by second surgery.
