Intertrochanteric fracture is a common clinical fracture.There are many classifications based on anatomical descriptions and prognosis.The current preferred treatment for Intertrochanteric fracture is internal fixation,which can be divided into extramedullary and intramedullary fixation systems.Each type has its advantages and disadvantages.Furthermore,there are still some problems that need to be resolved in terms of the timing of surgery,neck screw position,main nail length and distal locking mode,and perioperative hemorrhage.This article summaries the classifications of intertrochanteric fractures and the progress of internal fixation treatment,also discusses the issues that occur during perioperative period.Because of good judgment of stability and prognosis,Evans classification and Jensen-Evans classification are more clinically applied.Intramedullary nails represented by PFNA and Inter TAN gradually replaced extramedullary fixation due to better mechanical characteristics.Intertrochanteric fracture should be performed within 48 hours.Use full length intramedullary nails and static locking distal ends as far as possible and neck screws should be located below the center.Implicit blood loss is the main blood loss during the perioperative period and can not be ignored.
