Objective: We aimed to further determine the effectiveness of high-flow nasal cannula(HFNC)in comparison to noninvasive positive pressure ventilation(NPPV)and conventional oxygen therapy(COT)on adult patients after extubation.Methods: PUBMED,EMBASE,OVID,MEDLINE,Cochrane Library,CNKI,WANFANG,CBMdisc and VIP databases were searched for relevant publications in English and Chinese which were published during January2001~December 2017.Qualified clinical researches were chosen according to the selection and puts forward standard which we do quality evaluation and data extraction by.We choose the type of effect model on the basis of the heterogeneity inspection results to combine data to confirm the relationship between the evaluation of adult patierts with reintubation rate,ICU mortality and length of ICU stay,test the publication bias by funnel plot by Revman5.3.Results:Nine literatures were finally included for meta-analysis which totally contain 1983 patients.No significant heterogeneity was found in outcome measures.Compared with COT,HFNC was correlated with lower rate of reintubation(Z=2.99,P =0.003),and there is no statistical significance compared with NIPPV(Z=0.43,P =0.66).As for the ICU mortality and length of ICU stay(ICU LOS),we did not find any advantage of HFNC over COT or NIPPV.Conclusion: Compared with COT,HFNC can reduce the rate of reintubation in adult patients and can be used as an alternative to oxygen therapy for NPPV.
