Postoperative cognitive dysfunction(POCD)is a common complication after clinical anesthesia surgery,mainly presented as attention deficit,impaired memory,learning disability and damaged thinking language.A large number of gerontal patients have postoperative delirium or postoperative cognitive dysfunction,and closely linked with the forward dementia.Currently,sevoflurane as the ideal inhaled anesthetics has been widely used in clinic and easily accepted by patients of all ages.It is characterized by low blood/gas partition coefficient(B/G),stronger depth of anesthesia control,and a certain degree of muscle relaxation effect.It can be quickly induced and revived in anesthesia.As such widely-used central nervous system drugs,the safety of drug use has become the focus of attention in the process of promoting its application.A number of studies have confirmed that inhaled anesthetics like sevoflurane do have neurotoxic effects especially for gerontal patients,and at the same time it raises the risk of postoperative cognitive dysfunction.So far,the mechanism of intravenous anesthetics and inhaled anesthetics is not fully clear.Sevoflurane mediated neurotoxicity reaction by GABA receptor and NMDA receptor,and restrained the expression of muscarinic acetylcholine receptors to influence the plasticity of synapse.It was reported that sevoflurane can cause Endoplasmic reticulum stress(ERS)in brain tissue of aged rat and apoptosis of neuronal degeneration denaturation,so as to give rise to postoperative cognitive dysfunction.The endoplasmic reticulum stress of the brain and the cognitive part needs further research,and its correlation with sevoflurane should be confirmed by further study.Objective: The purpose of this research is to further explore whether the endoplasmic reticulum stress is associated with the postoperative cognitive dysfunction,whether sevoflurane has damage effect on temporal lobe artificial neuron in aged rats and correlation with postoperative cognitive dysfunction.Methods:1.The experiment selected 60 satisfactory male healthy aged rats(SD),which were divided into 5 random groups,12 in each group.The five groups are control group(C),2 hours and 3% sevoflurane postoperative day group(STO),2 hours and 3% sevoflurane in seven days group(STS),4 hours and 3% sevoflurane postoperative day group(SFO),and 4 hours and 3% sevoflurane in seven days group(SFS).Each group was trained of five-day water maze,and then the rats would take anesthesia.STO,SFO in drug delivery in one day would take Morris directional navigation experiment and space exploration experiment.STS,SFS in drug delivery in 7 days would take Morris directional navigation experiment and space exploration experiment.The latent time of escaping rats and times through the original platforms would be measured respectively.2.After the Morris test,6 rats in each group were randomly chosen to take Quantitative real-time PCR(RT-qPCR)to test gene transcription levels of Bip、ATF6、CHOP and the expression of Bip、ATF6、CHOP.Results:1.Compared with the control group,the latent period in the STO,SFO and SFS group(P<0.05)extended,and the STS group is no significant difference(P>0.05)2.Compared with the control group,times through the original platforms in the STO,SFO and SFS group decrease significantly(P<0.05),and the STS group is no significant difference(P>0.05)3.Compared with the control group,Bip,CHOP immunofluorescence density values in the STO,SFO and SFS group increase(P<0.05),and the STS group is no significant difference(P>0.05).Compared with the control group,ATF6 immunofluorescence density values in the SFO group increase(P<0.05),and in the STO,STS and SFS group is no significant difference(P>0.05).4.Compared with the control group,Bip,CHOPmRNA transcriptional level in the STO,SFO and SFS group increase(P<0.05),and the STS group is no significant difference(P>0.05).Compared with the control group,ATF6 mRNA transcriptional level in the SFO group increase(P<0.05),and in the STO,STS and SFS group is no significant difference(P>0.05).Conclusions:1.3% inhalation of sevoflurane for 4 hours and inhalation for 2 hours had a greater impact on cognitive function and lasted longer.2.The high solubility of sevoflurane inhaled for different time can cause Bip,ATF6,CHOP expression level increase to varying degrees.Memory disability caused by sevoflurane may be related with expression of Bip、ATF6 and CHOP in temporal lobe.
