The Influence Of RENAL Score And Other Related Factors On The Time Of Warm Ischemia In The Retroperitoneal Laparoscopic Nephrectomy

Objective: To investigate the influence factors of heat ischemia time in laparoscopic renal partial nephrectomy to ruduce bleeding time.Methods: Retrospective study on the imaging and clinical data of 87 cas es of renal tumor patients who underwent laparoscopic renal partial nep hrectomy from September 2012 to May 2017.According to the preoperative RENAL enhancement CT,RENAL score and the other parameters(The numbe r of Trocar,age,gender,smoking and drinking habits,benign and malig nant tumor)were combined,univariable analysis were used to investigate the relationship between the parameters and Whether the renal warm isc hemia time is greater than 30 min.Multivariate logistic regression was used to analysis determined independent risk factors.Transactional ana lyze was used to analysis the interaction of independent risk factors.Results:Result from univariable analyse shows that there were no significant difference in the scores of R,E,A and L in the RENAL score system,age,gender,smoking and drinking habits,and the pathology of the tumor between patients Whether the renal warm ischemia time is greater than30 min.Multivariate logistic regression shows that RENAL score,N single score and the number of Trocar were independent risk factors of Renal warm ischemia time greater than 30 min.Transactional analyze showed that the number of Trocar was interactive with RENAL score and N single score.Conclusion: The selection of the amount of Trocar in RENAL score and the combination of N single score will affect warm ischemia time in the posterior laparoscopic nephrectomy.According to preoperative evaluation,warm ischemia time can be reduced by appropriate surgical method.When RENAL score is greater than 7 or N less than 7mm,4 Trocar can be used to shorten the warm ischemia time in the operation.
