Objective To analyze the clinical data of very low birth weight infants(VLBWIs)and extremely low birth weight infants(ELBWIs)(birth weight ?1200 g)and to explore the clinical characteristics of perinatal period and its relationship with the complications of premature infants.Methods A retrospective analysis of the clinical data,treatment and in-hospital outcome of VLBWIs and ELBWIs,Shengjing hospital from Jan.1st 2011 to Dec.31st 2016.The same weight preterm infants were selected as the control group.We compared the characteristics of both maternal and infants,To analyze the factors affecting the prognosis of premature infants.Results There were 474 cases,158 cases(case group)and 316 cases in the control group.The rate of older women(17.7%),pregnancy-induced hypertension(23%),placental abnormality(20%),and cesarean delivery(57%)were significantly lower in the twin group than in the control group(25.9%,62%,33%).And 73%),P<0.05;initial production(53%),assisted reproduction(47%)and cervical cerclage(16%)were significantly higher than the control group(35%,12%and 7%),P<0.05.The gestational age of premature infants(28.9±2.3w)in the twin group was significantly lower than that in the control group(29.4±1.9w),P<0.05;5minApgar score[8(8,9)]was significantly lower than the control group[9(8,9)],P<0.05;The incidence of SGA(42%)was significantly lower than that in the control group(56%),P<0.05;The risk of PS utilization and occurrence of RDS was significantly higher than that in the control group(OR 2.904,2.29,P<0.05)..The male 1min Apgar score of the twin group[6(5,8)]was lower than that of the girl[7(6,8)],P<0.05;The rate of recovery(45%),EOS(27%)and BPD(45%)were significantly higher than that of girls(26%,9%and 26%),P<0.05.The average gestational age(28.3±2.2w)of the case group was significantly lower than that of the natural conception group(29.6±2.2w),P<0.05;The incidence of SGA(31%)was also significantly lower than that of natural conception group(51%),P<0.05;Respiratory support(91%)and BPD incidence(45%)were significantly higher than that in the natural pregnancy group(79%,29%),P<0.05;The birth weight(1019.3±2.1g)was significantly greater than that in the preterm infant(904.3±187.1g),P<0.05.There was no statistically significant difference between the two groups.Conclusion The preterm infants of twins had a small gestational age,and the risk of RDS and the application rate of PS were higher than that control group.There was no difference in the risk of complications between the two groups of preterm infants.The gender and the way of conception of twin preterm infants affected their hospital outcomes,and the chorionicity and birth order had no effect on the hospital outcome.
