Study On Relationship Between Biomarkers Of Lectin Pathway Of Complement And Type 2 Diabetic Kidney Disease

Objective: The study is to determine the serum levels of ficolin-3,MASP-2,MAp44 and C5b-9 in type 2 diabetes mellitus patients with different urinary albumin excretion and to explore the relationship between these biomarkers and type 2 diabetic kidney disease,which is helpful to us to understand the mechanism of the type 2 diabetic kidney disease.Methods: 1.All subjects were divided into three groups.Group A,a total of 41 healthy subjects,was the control group.178 type 2 diabetic patients with normal urinary albumin excretion(albumin-to-creatinine ratio < 30mg/g)were choosen as Group B and 86 type 2diabetic patients with increased urinary albumin excretion(albumin-to-creatinine ratio ?30mg/g)were choosen as Group C.2.Serum concentrations of ficolin-3,MASP-2,MAp44 and C5b-9 were determined with enzyme-linked immunosorbent assay(ELISA).3.Correlations between ficolin-3,MASP-2,MAp44,C5b-9 and type 2 diabetic kidney disease were analyzed by the comparisons between groups and the logistic regression analysis.Results: 1.Serum levels of ficolin-3 in group C(22.94±11.00 ?g/m L)and group B(21.70±8.91 ?g/mL)were higher than group A(13.47±6.02 ?g/m L),P < 0.05.serum level of MASP-2 in group C(98.74±50.47 ng/mL)was lower than group A(148.65±56.46 ng/m L)and group B(134.27±57.32 ng/mL),P < 0.05.Serum level of MAp44 in group C [2.08(1.32-2.83)] was higher than group A [1.59(1.31-1.82)] and group B [1.67(1.33-2.12)],P < 0.05.Serum levels of C5b-9 were not statistically different between three groups.The ratio of MASP-2 and MAp44 in group C[0.05(0.03-0.08)] was lower than group A [0.10(0.06-0.13)] and group B[0.08(0.05-0.12)],P < 0.05.2.Logistic single-factor regression analysis showed that age,duration of diabetes,SBP,FPG,Cys-C,eGFR,MASP-2,MAp44,C5b-9 and MASP-2/MAp44 were associated with type 2 diabetic kidney disease.3.Logistic multi-factor regression analysis showed that duration of diabetes(OR=1.059,95%CI=1.008-1.113,P=0.023),FPG(OR=1.152,95% CI=1.048-1.267,P=0.003),Cys-C(OR=9.846,95% CI=1.254-77.350,P=0.030)and MASP-2/MAp44(OR=0.002,95%CI=0.000-0.427,P=0.024)were independent variables for type 2 diabetic kidney disease.Conclusion: There are changes of lectin pathway-associated biomarkers MASP-2 and MAp44 in serum of patients with type 2 diabetic kidney disease,and MASP-2/MAp44 is one of the independent variables for type 2 diabetic kidney disease,indicating that activation of the lectin pathway of complement appears to be involved in the onset of type 2 diabetic kidney disease.
