Objective:To To evaluate the diagnostic value of ATP stress 99mTc-MIBI gated myocardial SPECT imaging in myocardial infarction.Methods:73 patients with previous history of myocardial infarction or suspected myocardial infarction were selected.All undergo vectorcardigram,cardiac color Doppler ultrasound,ATP stress 99mTc-MIBI gated myocardial SPECT imaging.If the radionuclide distribution of moderate to severe defects appeared on the anterior or inferior wall in ATP stress gated myocardial SPECT imaging,at least 2 or more than 2 segments of each site were still distributed or partially improved,then it was judged to be a myocardial infarction.Based on coronary angiography or coronary CTA,the sensitivity,specificity and accuracy of ATP stress myocardial perfusion imaging,vectorcardigram and color Doppler echocardiography were calculated and the diagnostic value of the three methods for old myocardial infarction was compared.Results:the sensitivity,specificity and accuracy,positive predictive value and negative predictive value were 82.6%,81.5%,82.2%,88.4%,73.3%respectively.Stress myocardial perfusion imaging and echocardiography and vectorcardigram,results show that the sensitivity and accuracy are significantly different between the three groups(P<0.05),the stress myocardial perfusion imaging sensitivity and accuracy is higher than that of echocardiography(P<0.01),and it's sensitivity is higher than vectorcardigram(P<0.01),but showed no significant diference in accuracy.There was no significant difference between the three groups of stress myocardial perfusion imaging,echocardiography and vectorcardigram in specificity.The diagnostic value of ATP tress myocardial perfusion imaging and echocardiography and vectorcardiogram in the three groups of old inferior myocardial infarction is consistent with the above.The diagnostic value of inferior myocardial infarction is slightly lower than anteroseptal myocardial infarction by myocardial perfusion imaging,but showed no significant difference between the two groups.Conclusion:ATP strss 99mTc-MIBI gated myocardial SPECT imaging is of important clinical value for the diagnosis of old myocardial infarction.
