Objective: The goal of the study is to identify risk features related to recurrence of chronic subdural hematoma(CSDH)treated by a novel twist-needle craniostomy(TNC).Methods: We retrospectively analyzed 61 patients with 74 CSDHs who underwent double-needle TNC.The CT density,MRI T1,and T2 intensity of the hematom were quantified by using a software.The quantitative analysis was used to examine the relationship between hematoma recurrence and various clinical and radiological features.Results: Eleven CSDHs experienced recurrence.Multivariate logistic regression analysis revealed that the age(OR=1.57,95% CI 1.066-2.312),MRI T1 intensity(OR=0.994,95% CI 0.988-0.999),and volume of hematoma(OR=1.038,95% CI1.007-1.069)were independent risk factors of recurrence.Furthermore,the hematomas with hypo-T1/hypo-T2 or hypo-T1/ slight hyper-T2 signals in MRI seemed to recur more likely.No relation was found between radiological features and hematoma evacuation rate.Conclusions: The double-needle TNC is a safe and effective procedure to treat CSDH.Treated by this procedure,patients with old age,low hematoma MRI T1 intensity,or high hematoma volume may possess a greater risk of recurrence.
