The TM6SF2 Gene Polymorphism And Nonalcoholic Fatty Liver Disease: A Meta-analysis

Purpose: Non-alcoholic fatty liver disease(NAFLD)is the most common chronic liver disease in the world with diverse disease spectrum and complicated etiology and pathogenesis.Its susceptibility and metabolic syndrome,environment,lifestyle,genetic factors are related.The relationship between TM6SF2 gene polymorphism and NAFLD is receiving more and more attention.This article mainly evaluates the susceptibility of TM6SF2-rs58542926 and NAFLD and the effects of liver enzymes,liver fat,blood lipid,fasting blood glucose and HOMA-IR in patients with NAFLD.Methods: We searched Pubmed,Embase,Wanfang Database,CNKI for prospective and retrospective studies of TM6SF2 and NAFLD before January 1,2018.Based on inclusion and exclusion criteria,the literature was rigorously evaluated and screened,data were extracted,and the relationship between TM6SF2-rs58542926 and NAFLD was studied by using the fixed and random effects models using odds ratios OR and 95%confidence intervals,and WMD and 95% confidence intervals Susceptibility and NAFLD patients with liver enzymes,liver fat,blood lipids,fasting blood glucose,HOMA-IR and other levels.Heterogeneous chi-square test and I2 statistics were used to draw Galbrath plots.Funnel chart for publication bias analysis.A single study on the impact of the combined effect of sensitivity analysis.Results: 14 articles were included in the analysis,a total of 6144 people.TM6SF2-rs58542926 was associated with NAFLD susceptibility(OR = 1.887,95% CI(1.581,2.250))with no heterogeneity and published bias,with a stable conclusion.Liver enzymes,TM6SF2-rs58542926 was associated with elevated ALT(WMD = 5.552,95%CI(0.071,11.033)),elevated AST(WMD = 2.860,95% CI-0.010,5.731)(WMD =-10.80,95% CI(-56.97,35.37)),but the research heterogeneity is larger and the research results are not stable.After removing the small sample,the heterogeneity decreases and the conclusion deflection is And AST,GGT increased related.In addition,the SNP was also associated with an increase in liver fat content(LFAT)(WMD = 6.574,95% CI(2.951,10.197))in NAFLD patients.In terms of lipids,TC(WMD =-0.214,95% CI(-0.336,-0.091))LDL-C(WMD =-0.112,95% CI(-0.200,-0.023));unrelated to HDL-C(WMD=0.005,95% CI-0.019,0.029)Research is heterogeneous.Finally,there was no significant correlation between TM6SF2-rs58542926 and BMI,fasting plasma glucose and HOMA-IR in patients with NAFLD.Conclusion: NAFLD is a multifactorial result.Among them,TM6SF2-rs58542926 mutation is associated with the incidence of NAFLD and liver fat content.In addition,patients with NAFLD with this gene mutation have lower levels of TG,TC and LDL-D.
