Background and objective:Fungal infection cause a variety of deck or under armour disease known as onychomycosis,pathogenic bacteria often include dermatophytes,saccharomycetes and non dermatophytes moulds,which caused by dermatophyte onychomycosis also known as onychomycosis.At present,the common treatment methods on the market include oral,external use and surgical treatment,but these methods have some disadvantages and shortcomings.With developed of laser technology,ultrapulsed CO2 laser is now widely used in the treatment of acne,keloid,photoaging and other department of Dermatology diseases.Ultrapulse CO2 laser can precisely control the depth of penetration of the skin and punctate exfoliation,and its thermal effects can inhibit fungi and activate the local immune response,and assist drug penetration absorption.In onychomycosis study,it found that serum in patients with onychomycosis in some cytokines such as IL-6,TNF-? expression increased.To establish an animal model of onychomycosis,ultrapulse CO2 fractional laser irradiation and combined with external use of amorolfine liniment on rabbit model of onychomycosis,pathologic changes and immunohistochemical interleukin cells before and after infection and treatment evaluation of IL-6,TNF-? expression.To explore the therapeutic mechanisms of disease.Experimental animals and methods:1,8 male Japanese white rabbits,with intramuscular injection of steroid hormones,Trichophyton infect rabbit,build model.2,rabbit model of onychomycosis were randomly divided into 4 groups,each group of 12 nails.Laser and medicine group,alone laser group and alone medicine group.Laser irradiation using super pulse CO2 laser Active FX mode: 200 HZ frequency,pulse delay3 S,energy density of 25J/cm,1 times a week for a total of 2 times;amorolfine 1 times a week for a total of 4 times,the combined treatment group for laser irradiation 1 times every 2 weeks,and 1 times a week painting;model group: no treatment.The course of treatment was 28 days.3.To observe the changes of pathological features of rabbit model through PASS staining of onychomycosis.4.immunohistochemical detect IL-6,TNF-?expressions of rabbit models of onychomycosis in different groups after treatment.5.The difference of four groups was compared with the single factor analysis of variance.The difference of the treatment group and the model group was compared with the Dunnett-t test.Result:1.With intramuscular injection of steroid hormones,Trichophyton infection of rabbit model was constructed successfully.After 2 weeks of infection,white and yellowish flaky changes were seen in the proximal part of rabbits.After removal of gauze and toe sleeves,the distal toenail changed after 4 weeks of infection,and could maintain the whole experiment to the end.2.The histopathological characteristics of rabbits of onychomycosis: in a large number of hypha can be seen in the deck,some hyphae reach nail bed.some small cataclastic hypha can be seen in the deck;surface are visible clusters of spores.3.the results of immunohistochemistry observation: microscopically,onychomycosis model group IL-6,TNF-? are massive expresions in rabbit nail bed,mainly in the epidermis and shallow layer of the dermis keratinocytes,vascular endothelial cells.The treatment group in the individual laser group and single group express visible positive cells significantly,but was lower than that of model group;IL-6,TNF-?positive cells of laser and medicine group significantly decrease,dyeing shallow light,weakly positive expression.Through the statistical analysis between the four groups,the treatment group and model group,medicine group and laser combined with medicine group,laser group,medicine group P < 0.05,have differences and statistical significance;alone laser group and alone medicine group P>0.05,have no difference and statistical significance.Conclusion:1.rabbit onychomycosis model was constructed successfully.2.ultrapulse CO2 laser irradiation,topical amorolfine liniment combined treatment can reduce the secretion of IL-6 and TNF-?in rabbit model of onychomycosis nail bed,the combined treatment group is better than the single treatment group;no significant difference between the single laser group and individual medicine group in the experimental parameters.
