Objectives:In this project,we study variations in genital dimensions of Chinese female,so that we can establish the database of normal female genital appearance living in Nanyue district,explore the relationship between the anthropomorphology of external genitalia and demology characteristic.We also tested the hypothesis of human adipose-derived stem cells(ADSCs)by applying the idea of stem cells to FGCS,to study the effect and potential mechanism by testing different concentration of estrogen on growth proliferation in vitro and differentiation into epidermal cells.Methods:1.Vulva measurement: 338 premenopausal women having gynaecological procedures not involving the external genitalia under general anaesthetic between September 2016 and June2017 were recruited in our study.Data included four morphology index,fifteen measurable parameters and other demographics.2.The effect of estrogen on growth proliferation: The forth generation of ADSCs were divided into 7 groups according to the concentration of estrogen : 10-6,10-7,10-8,10-9,10-10,10-11 mol/L,while estrogen was not added to the control group.Then methods cell counting kit-8(CCK-8)was used to examine the effect of different concentrations of estrogen on growth proliferation of ADSCs.3.The effect of estrogen on differentiation: The fifth generation of ADSCs were divided into 6groups according to the concentration of estrogen : 10-6,10-7,10-8,10-9,10-10 mol/L,while estrogen was not added to the control group.As for the adipogenic differentiation of ADSCs,we stained with oil red O at 14 days after induction and differentiation,and then compared the discrepancy in ability of adipogenic differentiation among the different concentration of estrogen.To explore the ability of epithelial Differentiation of ADSCs,western blot is used to detect the expression of cytokeratin 19 in each group.Results:1.The primary morphology index were as follows: most of labia minora appeared oval shape(n= 218,64.5 %)and a majority of clitoral glans were embedded in the clitoral hood(n=200,59.2%);as for labia majora,237 cases had normal appearance,while 19 cases were hypertrophic and 82 cases were ptotic or deflated.The primary parameters were as follows: left labia minora length(44.10 ± 9.57)mm,right labia minora length(43.51 ± 10.20)mm,left labia minora width(15.17 ± 6.10)mm,right labia minora width(14.56 ± 6.25)mm,left labia minora thickness(3.89± 1.32)mm,right labia minora thickness(3.92 ± 1.25)mm,labia majora length(82.52 ± 8.55)mm,left labia majora width(22.41 ± 3.14)mm,right labia majora width(22.46 ± 3.18)mm,the clitoral glans length(4.53 ± 1.92)mm,clitoral glans width(3.12 ± 1.34)mm,clitoral hood length(24.08 ± 5.29)mm.There was a positive correlation between both left and right sides of labia minora width and vaginal delivery times(P < 0.05).The length and width of labia majora were associated with weight(P < 0.01).There was a positive correlation between distance and weight,which was from clitoris to vagina,urethra to vagina,vagina to perineum and perineal body length(P < 0.05).2.The proliferation ability of ADSCs was statistically significant under different concentrations of estrogen(F = 3.378,P = 0.007).The most suitable concentration had been tested in our study is 10-9 mol/L.3.After 14 days induction and differentiation,the result of oil red O showed that estrogen of certain concentration could promote the adipogenic differentiation.The use of 10-8mol/L and10-9mol/L estrogen increase the amount of ADSCs most(P < 0.01).The result of western blot showed that estrogen inhibited epithelial differentiation of ADSCs.Besides,different concentrations of estrogen showed different strength of inhibition.Conclusion:1.The appearance and measurements of female external genitalia varied individually,especially labia minora.Some of the measurements appears to be associated with age,weight and labor history.2.Estrogen promotes the proliferation of adipose-derived stem cells relating to its concentrations.The most suitable concentration had been tested in our study is 10-9 mol/L.3.The stimulation of a certain concentration of estrogen can promote the adipogenic differentiation of ADSCs.On the contrary,the use of estrogen 2 inhibited ADSCs to differentiate into epidermal cells,which was associated with the concentration of estrogen.The ability of epithelial Differentiation was significantly inhibited when the concentration of estrogen was too high or too low.
